purpose:To explore the predictive value of cystatin C in the early diagnosis of diabetic kidney disease,looking for early warning value of diabetic kidney disease,so as to provide early warning for the early detection of diabetic kidney disease.Method:This study included 300 patients with type 2 diabetes who were admitted to the Department of Endocrinology and Metabolism of the First Hospital of Jilin University from August 2016 to August 2018.We collected general information and laboratory test data.?1?All the patients were divided into two groups according to glomerular filtration rate(eGFRMDRD):Group A is eGFRMDRD<60 ml/min/1.73m2,group B is eGFRMDRD?60 ml/min/1.73m2,then we compared the differences of the above indicators between the two groups,analyzed the correlation between glomerular filtration rate and clinical indicators,and drew ROC curves of urine microalbumin and cystatin C,comparing the area under the curve?AUC?.?2?All the patients were divided into three groups according to the 24-hour urine microalbumin:group a is simple diabetes group?24-hour urine microalbumin<30mg/24h?,group b is early diabetic nephropathy group?24-hour urine microalbumin is 30-300mg/24h?,group c is the clinical diabetic nephropathy group?24-hour urine microalbumin>300mg/24h?.Then we compared the differences of various kidney indicators between the groups,analyzed their correlation with urine microalbumin,and used the ROC curve to evaluate the diagnostis value of cystatin C for early diabetic kidney disease.Result:?1?Compared with group B,cystatin C in group A was significantly increased,and the glomerular filtration rate was negatively correlated with cystatin C,and the correlation was strongest?r=-0.860,P<0.001?,multiple factors Logistic regression analysis showed that cystatin C is an independent risk factor for decreased renal function.The area under the ROC curve of cystatin C?AUC=0.952?was larger than the area under the ROC curve of urine microalbumin?AUC=0.794?.?2?On comparison between group a and group b,only cystatin C was significantly increased?P<0.05?.Urinary microalbumin was positively correlated with cystatin C levels.The areas under the curve of cystatin C in early diabetic kidney disease and clinical diabetic kidney disease were 0.774 and 0.797,respectively,and the critical values were0.88 mg/L and 1.03 mg/L,respectively.In conclusion:For the early prediction of diabetic kidney disease,cystatin C has good diagnostic efficacy and may be used as an early warning value for diabetic kidney disease.
