Objective:Through the investigation and research on the candidates registered for the national medical qualification examination of the category of clinical practitioners,to understand the basic situation,work,training and assessment,employment intention and other information of the candidates,and to follow up and study the examination results and total scores of the three dimensions of the candidates' practical skills examination of doctors' qualification,and analyze the factors affecting the passing of the examination of practical skills of doctors' qualifications.To understand the relationship between examinees' professional skills and examination results,to provide support for the ability training of medical students,the improvement of medical teaching system and the standardized training of residents,and to provide support for the reform and development of national physician qualification examination.Methods:A questionnaire survey was conducted on the candidates who participated in the clinical category of the 2019 National practical skills examination for Doctor qualification in Changchun examination Center of Jilin examination area.the contents include the basic situation,work unit,training and assessment,employment intention and so on.A total of 2206 questionnaires were distributed and 2200 were recovered,of which 1928 were valid,with a recovery rate of 99.72% and an effective rate of 87.63%.Epidata3.1 was used for data input and IBMSPSS24.0 statistical software for statistical analysis,quantitative data were expressed as(relative X ąS),qualitative data were expressed as relative numbers,quantitative data obeying normal distribution were tested by t-test and analysis of variance,qualitative data were tested by test,and quantitative data that did not obey normal distribution were tested by Kruskal-Walls H test.Multivariate Logistic regression analysis was used to analyze the relationship between passing the examination and basic conditions,participating in standardized training of residents,clinical working years and professional interest.Result:1.The proportion of men and women under 25 years old is about 44.5%,undergraduate degree is 80.3%,working in tertiary medical institutions is 64.7%,working in clinical department is 68%,working for one year or less is 50.4%.68.6% of them are taking part in standardized training for residents or have completed training.Interest in clinical work accounted for 98.8%.As a whole,the examinees are mainly highly educated,younger and clinical front-line workers,and most of them are participating in or have participated in the standardized training of residents.2.The first dimension of the practical skills examination of physician qualification mainly examines the examinee's ability of consultation and doctor-patient communication.Univariate analysis was carried out for the scores of the first dimension of candidates with different characteristics,the results showed that there were significant differences in the first dimension scores of all variables among candidates of different ages,majors,educational background,level of work unit,working years,standardized training of residents,frequency of unit skill examination and work interest(P < 0.05).3.In the investigation of the second dimension,the clinical operation ability of the examinee is mainly examined through physical examination and basic skills operation,which is the focus of the practical skill examination of physician qualification.In the second dimension,the scores of candidates with different characteristics were analyzed by single factor analysis.The results showed that there were significant differences in the second dimension scores among different ages,majors,levels of work units,working departments,clinical working years,standardized training of residents,skills training of units,assessment of skills of units,clinical practice opportunities,and those who liked theory or practice and practice and theory ability(P < 0.05).4.The third dimension examines the examinee's ability of auxiliary imaging examination and the professional quality of doctors.Univariate analysis was conducted through the scores of the third dimension of candidates with different characteristics,the results showed that there were significant differences in the third dimension scores of candidates with different ages,majors,levels of work units,working departments,clinical working years,standardized training of residents,unit skills training,clinical practice opportunities,like theory or practice and practice and theoretical ability(P < 0.05).5.In the univariate analysis of the total scores of candidates with different characteristics,there were significant differences in gender,age,major,level of work unit,current work department,clinical working years,standardized training of residents,frequency of unit skill training,unit skill assessment,clinical practice opportunity,like theory or practice and practice and theoretical ability(P < 0.05).Through Logistic regression analysis,it is concluded that the number of years engaged in clinical work,participation in standardized training of residents,clinical practice opportunities,examinee age,professional interest and major are statistically significant.Conclusion:1.In the basic situation,age and major have the greatest influence on the examinee's performance in the three dimensions,and they are the key factors that affect the examinee's performance.2.In the assessment of the three dimensions,participating in the standardized training of residents has a great impact on the examination results of candidates.The examinees who have participated in the standardized training of residents or are taking part in the standardized training of residents have ideal examination results,which also shows the importance of standardized training of residents in improving the examination ability of candidates.3.The number of years of clinical work and the number of practical opportunities have an impact on the examinee's scores in the three dimensions.This shows that adequate clinical practice is the key factor for candidates to pass the examination smoothly.4.In the process of preparing for the practical skills examination of physician qualification,candidates should prepare for both theory and practice at the same time.5.Most candidates are willing to devote themselves to clinical front-line work after passing the doctor qualification examination,but the current tense doctor-patient relationship is their biggest ideological obstacle,and the government,society and schools should pay attention to improvement.
