Non-alcoholic fatty liver disease(NAFLD)is a liver disease in which hepatocyte fat accumulation,inflammation,and fibrosis occur without excessive alcohol consumption.Currently,The diagnosis of nonalcoholic fatty liver disease,especially nonalcoholic steatohepatitis(NASH)lacks an effective and simple method.With the research on the pathogenesis of nonalcoholic fatty liver disease,more and more serological markers were used to a non-invasive diagnosis method for non-alcoholic fatty liver disease.Partial exploration of the diagnostic value of serum biomarkers for nonalcoholic fatty liver disease,especially nonalcoholic fat to hepatitis.This study will erpore the diagonosis value of serum biomarkers for nonalcoholic fatty liver disease,especially nonalcoholic steatohepatitis.1.Diagnostic significance of serum PTX3,CK18 and FGF21 for nonalcoholic fatty liver diseaseObjective : To evaluate the diagnostic value of serum Pentraxine3(PTX3),cytokeratin 18(CK18)and fibroblast growth factor 21(FGF21)in nonalcoholic fatty liver disease,especially nonalcoholic steatohepatitis.Methods:34 patients with NAFLD who were admitted to Shanghai Ninth People’s Hospital from July 2013 to August 2016 were enrolled as experimental groups.The corresponding liver tissue samples were collected.There were 21 non-NASH groups included 8 simple fatty liver and 13 borderline NASH and 13 NASH according to the histopathological.Healthy volunteers who participated in the physical examination at the same time as 8 control group.We expored the levels of serum PTX3,CK18 and FGF21 in each group and and assessed the diagnostic value for NAFLD,especially NASH by ROC curve.At the same time,anthropometric indicators and relevant biochemical examination data were collected for analysis.Results:The AUC values of serum CK18 and FGF21 in the diagnosis of NAFLD were 0.752 and 0.859,respectively.When the serum CK18 level was >91.907 U/L,the sensitivity of diagnosis of NAFLD was 67.65%,and the specificity was 75%.When serum FGF21 level was >164.039 pg/ml,the sensitivity of diagnosis of NAFLD was 76.47%,and the specificity was 87.50%.The AUC of the combined diagnosis of NAFLD was 0.886.Both serological markers have no significance for the diagnosis of NASH.Serum PTX3 has no diagnostic significance for both NAFLD/NASH.Conclusion: Serum CK18,FGF21 and their combination can effectively diagnose nonalcoholic fatty liver disease,and has no diagnostic significance for nonalcoholic steatohepatitis.2.the diagnostic significance of serum LCN2,PAI1 for nonalcoholic fatty liver disease.Objective:This study evaluated the diagnostic value of lipocalin 2(LCN2)and plasminogen activator inhibitor 1(PAI1)in nonalcoholic fatty liver disease, especially nonalcoholic steatohepatitis.Methods:82 patients with NAFLD who were admitted to Shanghai Ninth People’s Hospital from July 2013 to August 2017 were enrolled as experimental groups.The corresponding liver tissue samples were collected.50 non-NASH groups and 32 NASH groups were classified according to liver histopathological findings.The non-NASH gtoups included 20 simple fatty liver and 30 borderline NASH.Serum LCN2 and PAI1 levels were measured in each group,and the diagnostic value forNAFLD,especially NASH was evaluated by ROC curve.At the same time,anthropometric indicators and relevant biochemical examination data were collected for analysis.Results:The AUC of serum LCN2 and PAI1 for the diagnosis of NAFLD were 0.882 and 0.812,respectively.When the serum LCN2 level was >57.57 ng/ml,the sensitivity of NALLD was 76.47% and the specificity was 100%.When the serum PAI1 level was >9.71 ng/ml,the sensitivity of diagnosis of NAFLD was 70.73%,and the specificity was 100%.The AUC of serum LCN2 and PAI1 for the diagnosis of NASH were 0.868 and 0.954.When the serum LCN2 level was >84.485 ng/ml,the sensitivity of the diagnosis of NASH was 84.62%,and the specificity was 80.95%.When the serum PAI1 level was >11.60 ng/ml,the sensitivity of the diagnosis of NASH was 100% and the specificity was 90%.The AUC of the combined diagnosis of NASH was 0.971.Conclusions:Serum LCN2 and PAI1 can effectively diagnose NAFLD/NASH.
