Clinicalanalysis Of Benign Epilepsy Syndrome Within 1 Year Old

Objective: To investigate the diagnosis,treatment and course of treatment of benign epilepsy within 1 year old by observing the clinical features and following up of 63 children with epilepsy who had a good prognosis.Methods: Retrospective analysis of the clinical data of children with benign epileptic characteristics diagnosed and treated in the children's Hospital Affiliated to Chongqing Medical University from 2015 to 2018,and follow-up of the treatment course and prognosis.Results: Clinical features: the ratio of male to female was 1:1.25 in this group.There were 3 cases(4.8%)with onset age from 0 to 28 days,6cases(9.5%)from 28 days to 3 months,and 54 cases(85.7%)from 3months to 12 months,and the average age of onset was(5.8±2.9)months.20 cases(31.7%)had family history of convulsion or epilepsy.52 cases(82.5%)were generalized seizures,4 cases(6.3%)were focal seizures,7cases(11.1%)were focal progressing to bilateral tonic clonic seizures.48cases(76.2%)had cluster attack.The interictal EEG 47 cases(74.6%)showed normal,14 cases(22.2%)showed sharp / sharp slow wave in the period of sleep,and 5 cases(7.9%)showed background slowing in the period of awake.There are 5 of the 7 cases ictal EEG were of focal origin.There are 14 cases who have genetic testing,3 cases did not test the relevant pathogenic genes,and 11 cases reported PRRT2 gene mutations,including 10 base repeat mutations(c.649 dupc,p.r217 pfs * 8)and 1 base deletion mutation(c.623 delc,p.s208fs).Treatment and follow-up: 45 cases(71.4%)were treated with AEDs,8 cases with VPA,4 cases with OXC and33 cases with LEV.Among them,14 cases with LEV were poorly controlled.The shortest course of treatment was 3 days,the longest was 57 months,and the average course of treatment was(28.8±18.0)months.62cases(98.4%)stopped attack within 24 months,the average age of the last attack was(8.5±5.0)months.Conclusion: Most of the benign epilepsy patients within 1 year old are over 3 months old.The pattern of attack can be generalized,focal or focal progressing to bilateral tonic clonic seizures,and most of them are cluster attack;a majority of the interictal EEG are normal,a few of them have the background slowing down or the peak wave releasing in the period of sleep,and most of the ictal EEG are focal origin;great majority of them stop the attack within 2 years old and have good effect of single drug treatment.
