Objective: Left ventricular systolic function in breast cancer patients receiving Ta Regimen Chemotherapy was assessed using echocardiography and two-dimensional speckle tracking technique, respectively,to evaluate the advantages of two-dimensional speckle tracking technique in early diagnosis of left ventricular systolic function changes. The left ventricular diastolic function was measured by doppler technique in order to guide the early clinical measures. Methods: To collect 45 breast cancer patients who accepted Ta Chemotherapy in our hospital, all of them were female, and divided into chemotherapy group. All of them were found to have breast space-occupying lesions by x-ray or ultrasound, and then were punctured under the guidance of ultrasound, the pathological results confirmed. There was no significant difference between age and sex in 30 cases of Control Group. All patients were followed up by echocardiography(UCG),Doppler technique and 2-dimensional speckle tracking imaging(2D-STI)before chemotherapy,2 cycles after chemotherapy and 4 cycles after chemotherapy, the left ventricular function was measured.1、Assessment of the left ventricular diastolic function: The Pulse Doppler technique was used to measure the peak flow velocity(e peak,a peak)in the early and late stages of Mitral Valve Diastole, and e / A value was calculated automatically. In addition,Tissue doppler technique was used to measure the peak values(E’,a’)of myocardial motion in the early and late stages of Mitral annulus diastole.2、Assessment of left ventricular systolic function: A modified 2D Simpson biplane method was used to plot the four-chamber and two-chamber sections of the apex of the heart in the same cardiac cycle. The following results were calculated by a computer system.Left Ventricular End-Diastolic Volume(LVEDV),Left Ventricular End-Systolic Volume(LVESV)and Left Ventricular Ejection Fraction(LVEF).And at the same time, three consecutive stable cardiac cycles of four-chamber,three-chamber and two-chamber of left ventricular apex were selected by two-dimensional speckle tracking technique, the Left Ventricular Systolic Peak Global Peak Longitudinal Strain(LVGLS)and systolic peak global peak Longitudinal strain(LVGLS)of the Left ventricle were obtained.Rsults:1、Left ventricular diastolic function parameters:Doppler Technique: The Chemotherapy Group received 2 cycles of chemotherapy compared with the before chemotherapy,E / A,E’/ A’ had no significant difference(p > 0.05),E / A and e’/ a’ were decreased after 4 cycles of chemotherapy(p < 0.05).2、Assessment of left ventricular systolic function:(1)There was no significant difference in LVEDV,LVESV and LVEF between pre-chemotherapy and control group(p > 0.05).After chemotherapy, there was no significant difference in LVEDV,LVESV and LVEF between pre-chemotherapy and control group(p > 0.05).(2)Using two-dimensional speckle tracking: Compared with the control group,LS and LVGLS had no significant difference before chemotherapy(p > 0.05),LS and LVGLS had no significant difference after 2 cycles of chemotherapy(p > 0.05),LS and LVGLS had significant decrease after 4 cycles of chemotherapy,(p < 0.05).Conclusions: During Chemotherapy for breast cancer, chemotherapy drugs affect the left ventricular function of the patient.Two-dimensional speckle tracking technique, compared with the conventional echocardiography, early Detection of left ventricular systolic function in patients with breast cancer.
