Objective: To study the clinical characteristics,pathogen distribution,and infection parts of multiple myeloma Infectious patients in Qinghai,and the risk factors of the disease in this region,so as to provide strong basis for effective clinical control and prevention.Methods: The clinical data of patients diagnosed with multiple myeloma and co-infection in the Qinghai University Affiliated Hospital and Qinghai Provincial People's Hospital from January 01,2016 to November 1,2019 were collected,and is concluded by statistical analysis of the clinical data.Results:The median of 109 cases of multiple myeloma with infection was 60.00±10.41 years.A total of 109 cases met the criteria were selected.A total of 781 samples were sent for testing from 109 infection cases,of which 83 were positive,with a detection rate of 10.63%.Respiratory infections were the most common,accounting for 71.65% of the infection sites,with a higher incidence of lower respiratory infections(57.80%),followed by urinary tract infections,bloodstream infections,intestinal infections,abdominal cavity and skin infections.The infected pathogens were mainly Gram-negative(58.67%),and Escherichia coli was the main one in Gram-negative bacteria The bacteria were the most common(33.73% of all pathogens),the detection of Gram-positive bacteria was relatively small,accounting for 19.28%,Gram-positive bacteria were common Staphylococcus aureus(6.02%),the detection of fungi was 8.43%,the fungus was smooth bulb like yeast,accounting for 3.61%,while virus and other special bacteria infection was rare,accounting for only 3.61%.univariate analysis of dexamethasone cumulative measurement,neutrophil count,albumin level,globulin level,M protein level,age,invasive catheter placement,smoking history,associated chronic obstructive pulmonary disease,high-risk cytogenetic changes,body weight 11 factors such as index?24kg/m2 were statistically significant(p <0.05).again,multifactorial analysis suggested that granulocyte deficiency(or =1.794,p =0.009),hypoproteinemia(or =1.158,p =0.009),cumulative dose of high dexamethasone(or =0.998,p =0.014),invasive catheter placement(or =9.049,p =0.046)were independent risk factors for infection in patients with multiple myelomoma.Conclusion:1.The infection age of multiple myeloma patients in Qinghai is mainly middle-aged and elderly,the male is higher than the female,the disease stage is later,mostly stage II or above,and Ig G type was the dominant type.the site of post-infection infection in patients with multiple myeloma in this area is common in the respiratory tract,especially in the following respiratory tract,and the strain of infection is mostly Gram-negative bacteria.2.The patients with multiple myeloma Multi-factor analysis showed that neutropenia,hypoalbuminemia,high cumulative dose of dexamethasone and invasive catheterization for MM patients were independent and dangerous factors for getting infection.When these independent risk factors exposed,patients are required to be closely monitored to prevent infections,adopt early intervention,and reduce their physical mental and financial burden.
