Effect Of Apatinib On Renal Function In Patients With Advanced Gastric Cancer

Objective: To study and analyze the effect of taking apatinib on renal function in patients with advanced gastric cancer.Methods: A total of 57 patients with advanced gastric cancer who took apatinib at the Affiliated Hospital of Qinghai University from October 2017 to October 2019 were collected.According to the medication guidelines and individual differences,the subjects were divided into low-dose groups(23 individuals,250 mg/time)and the high-dose group(34 individuals,500 mg/time)were taken orally once a day for 28 consecutive days as a medication cycle.The data of the patient's basic signs,Karnofsky functional status score(KPS score),history of radiotherapy and chemotherapy,operation history,dosage and cancer metastasis were recorded in detail1 day before and 1 day after the end of medication.The blood pressure,urine routine,blood routine,biochemical examination and other data of patients before and after treatment were recorded,collected and analyzed statistically.According to whether the patients developed hypertension and proteinuria,they were divided into two groups.The patients were analyzed for hypertension and abnormal renal function after taking apatinib.Results: 1)Therapeutic effect of the patients: after 57 patients were treated with apatinib,the complete remission,partial remission,stable disease and disease progression were 3 cases,8 cases,18 cases and 28 cases,respectively.The objective remission rate was 19.30%,and the control rate is 50.88%;2)Comparison of KPS scores before and after treatment: before treatment,the average KPS score of the patient was 59.72 ± 8.47 points.After treatment with apatinib,the KPS score increased significantly,reaching 70.59 ± 9.03 points,the difference was statistically significant(p<0.05);3)Comparison of various biochemical indicators before and after treatment:After treatment,the number of patients with proteinuria was 33,accounting for57.89% of the total number.Albumin,alkaline phosphatase,direct bilirubin increased,urea nitrogen,urine,the specific gravity and urinary protein increased,and the creatinine clearance decreased,which was statistically significant compared with before treatment(p<0.05).Patients' blood routine,liver function,blood lipid and other indicators were significantly worse(p>0.05);4)Comparison of blood pressure before and after treatment: after treatment,a total of 23 patients developed hypertension,accounting for 40.35% of the total.Specifically,the average diastolic blood pressure and systolic blood pressure before treatment were 75.51 ± 8.29 mm Hg and 119.14 ± 10.42 mm Hg,respectively,and the average diastolic blood pressure and systolic blood pressure after treatment were89.08 ± 21.13 mm Hg and 140.29 ± 29.18 mm Hg,suggesting that the patient's blood pressure increased after treatment,and this difference was statistically significant(p <0.05).Conclusions: 1)After taking oral apatinib in patients with advanced gastric cancer,the disease remission rate and control rate are acceptable;2)Compared with before treatment,patients with advanced gastric cancer had higher urea nitrogen and decreased anhydride clearance after oral administration of apatinib.57.89% of patients had proteinuria,suggesting that patients had certain renal dysfunction;3)40.35% of the above-mentioned patients developed hypertension after oral administration of apatinib.Compared with before medication,their overall average systolic and diastolic blood pressure were significantly higher than before medication,the difference was statistically significant(p < 0.05),suggesting that apatinib has a risk of increasing blood pressure.
