Objective Through the study on the correlation between serum vitamin D(Vit D)level and children with primary nephrotic syndrome(PNS)and their bone metabolism,to explore the role of Vit D in the pathogenesis of PNS and provide a reference for treatment.Methods 68 patients with primary nephrotic syndrome in the General Hospital of Ningxia Medical University from October 2018 to December 2019 were selected as the study objects.Before hormone treatment,laboratory indexes such as serum 25-hydroxyvitamin D(25(OH)D)levels,red blood cells(RBC),hemoglobin(HGB),serum creatinine(SCr),serum albumin(ALB),total cholesterol(TC),24-hour urine protein quantitative(TP-U)and bone metabolism indicators such as alkaline phosphatase(ALP),calcium(Ca),phosphorus(P),and ultrasound bone density Z value were collected.Meanwhile,50 healthy children were selected as the control group.Compare 25(OH)D levels in children with PNS and normal children,and analyze the correlation between Vit D levels and related laboratory and bone metabolism indexes in children with PNS.All data were analyzed by SPSS 17.0,p < 0.05 was statistically significant.Results 1.The average age of children with PNS in this group was 5.03±3.67 years,including 23 infants(33.8%),25 preschool children(36.8%),and 20 school-age children(29.4%).48 males and 20 females,with a male to female ratio of 2.4:1.2.The 25(OH)D level in children with PNS was 9.59±6.85ng/ml,and the3.25(OH)D level in children in the normal control group was 30.17 ±11.13ng/ml.The level of 25(OH)D in children with PNS was lower than that in normal children(p < 0.05).3.According to the level of 25 hydroxyvitamin D,it was divided into three groups: vitamin D deficiency group,vitamin D insufficiency group and vitamin D normal group.In children with PNS,91.2% of them were vitamin D deficient and 8.8% were vitamin D insufficiency and 0% were vitamin D normal.In normal children,20% of them were vitamin D deficient and 36% were vitamin D insufficiency and 44% were vitamin D normal.4.After Pearson correlation analysis between serum 25(OH)D and PNS related laboratories and bone metabolism indicators: 25(OH)D level was negatively correlated with TC(r=-0.270,p < 0.05),Positive correlation with ALB(r=0.672,p<0.01),negatively correlated with TP-U(r=-0.253,p ? 0.05);negatively correlated with P(r=-0.289,p < 0.05),and correlated with Ca(r = 0.438,p < 0.01)and bone mineral density Z values(r=0.305,p<0.05).There was no significant correlation between 25(OH)D and SCr,RBC,HGB and ALP.Conclusion 1.The peak incidence of children with PNS in this group is mostly in the preschool age,and there are more men than women in children with PNS.2.The level of VitD in children with PNS is generally lower than that in normal children,and 91.2% of PNS children have VitD deficiency.3.The level of 25(OH)D in children with PNS was related to ALB,TC,TP-U,Ca,P,and bone mineral density Z values,and there was no significant correlation with SCr,RBC,HGB,and ALP.
