Effects Of Retroperitoneal Laparoscopic CO₂ Pneumoperitoneum On Cerebral Oxygen Saturation And S-100? Protein In Elderly Patients

Objective:To investigate the effect of CO₂ pneumoperitoneum under retroperitoneal laparoscopy on regional cerebral oxygen saturation and S-100?protein in elderly patients.Methods:Fifty-five elderly patients who underwent retroperitoneal laparoscopic radical nephrectomy from May 2018 to March 2019 were included,rScO₂ was monitored by FORE-SIGHT near infrared noninvasive brain oxygen detector.Pulse oxygen saturation?SpO₂?,heart rate?HR?,regional cerebral oxygen saturation?rScO₂?,and blood pressure?MAP?were recorded respectively at T₀?immediately after entering the operating room?,T1(5min before CO₂ pneumoperitoneum,T2?10min after CO₂ pneumoperitoneum?,T₃?30min after CO₂ pneumoperitoneum?,T4?60min after CO₂ pneumoperitoneum?,T₅?10min after CO₂ pneumoperitoneum was closed?.Immediate blood gas PH,PaO₂,PaCO₂,Hb and other data of T₀^T₅ were detected and recorded.The S-100?protein in plasma of internal jugular vein was detected immediately after T₀ and T₄.Results:Compared with T₀,rScO₂ at time T₄^T₅ was increased,and the difference was statistically significant?P<0.05?.Compared with T₀^T₁,the PH value at T₃^T₅ was significantly lower,and the difference was statistically significant?P<0.05?.Compared with T₀^T₁,PaCO₂ in T₂^T₅ was increased,and the difference was statistically significant?P<0.05?.Compared with T₀,s-100?protein was significantly increased at T₄,and the differences were statistically significant?P<0.05?.There was no significant difference in MAP,HR,SpO₂ between T₀ and T₅?P>0.05?Conclusion:Retroperitoneal laparoscopic CO₂ pneumoperitoneum significantly increased rScO₂in elderly patients.Moreover,s-100?protein and PaCO₂ in cervical venous blood increased gradually with the prolongation of CO₂ pneumoperitoneum.
