Objective:We analyzed the difference in the distribution of the genotype and alleles of rs222747 variants of the transient receptor potential vanilloid 1(TRPV1),serum concentration of substance P(SP)and calcitonin gene-related Peptide(CGRP),the incidence of intraoperative adverse cardiovascular events(ACVE)in the diabetic group and the non-diabetic group,to explore the correlation between TRPV1 gene polymorphism and genetic susceptibility to type 2 diabetes and intraoperative adverse cardiovascular event.Methods:one hundred and sixty-four patients undergoing elective knee replacement or posterior lumbar interbody fusion were assigned to two groups,diabetes group(DM group,n=86)and non-diabetes group(nDM group,n=78),according to the patients had suffered diabetes before operation or not,respectively.The preoperative general characteristics,gender,age,ASA grade,body mass index,blood lipid,preoperative blood glucose,intraoperative vital signs,operating time,anesthetic time,infusion volume were recorded.Real-time fluorescence quantitative PCR was carried out to genotype the variant rs222747 of TRPV1 gene.ELISA was used to determine the serum contents of substance P(SP)and calcitonin gene-related peptide(CGRP),to analyze the relationship between genotype and allele frequency and type 2 diabetes,the serum contents of SP and CGRP and intraoperative adverse cardiovascular events.Results:1.The difference of genotype and distribution frequency of allele between DM group and nDM group was statistically significant(P<0.05).2.The levels of serum SP and CGRP were significantly lower in the patients of DM group than that in nDM group(P<0.05).3.The incidence of intraoperative ACVE in the patients of DM group was significantly higher than that in nDM group(51.16% vs 29.49%)(P<0.05).4.According to the classification of the occurrence of ACVE in intraoperative period,there was a statistically significant difference in the overall frequency distribution of the TRPV1 gene variant rs222747 between the ACVE group and non-ACVE group(P<0.05).The SP content in the ACVE group was significantly lower than that in the non-ACVE group(P<0.05).5.According to the genotype classification of TRPV1 variant rs222747,SP and CGRP contents in the genotype CC,CG and GG groups were statistically significantly different(P<0.05).Conclusion:The polymorphism of TRPV1 gene variant rs222747 is related to the genetic susceptibility to T2 DM and the occurrence of intraoperative ACVE.The higher morbidity of intraoperative ACVE may be associated with the lower level of SP and CGRP.
