Objectives:Heart failure(HF)is a clinical syndrome that changes the structure within the heart leading to a reduction in cardiac output,mainly caused by myocardial infarction(MI).Only about 25% of heart failure symptoms occur after myocardial infarction Studies have shown that whether miRNAs in non-coding RNAs regulate this important pathophysiological process,and whether biomarkers with higher-cost performance can be found,miRNAs in exosomes are worth further exploration.The purpose of this article is to find circulating exosome miRNAs(plasma,serum and whole blood)that are related to the prognosis of heart failure after myocardial infarction.Method:In this article,we collected the ST-segment elevation myocardial infarction(STEMI)that received continuous percutaneous coronary intervention(PCI)from 2010 to 2013 collected by scholars in Warsaw,Poland(Maciejak A,Kiliszek M),and were followed up for six months(On the day of admission,the 4th-6th day,the first month,the sixth month)the expression profile chip data of the peripheral blood specimens from the patients,and the deep expression mining of the gene expression data.The gene with the top 25% of the coefficient of variation was selected as the model,and the weighted co-expression analysis(WGCNA)was used to obtain the node gene or hub gene.Finally,GO and signal pathway enrichment analysis were performed(KEGG),constructing the topological network and protein interaction network(PPI)of sample gene interaction,and after that understanding the potential biological characteristics of Biomarker associated with the prognosis and time of PCI surgery in STEMI patients.afterwards,based upon the chip data of patients with heart failure in the same cohort within six months,similar genes were compared to further screen out the node genes related to the prognosis of PCI.The miRNAs of the node genes predicted by TargetScan,miRDB,and mirwalk are verified in the existing exosome database(EV MiRNA,The Extracellular Vesicles miRNA Database),which is the basis for the next clinical patient plasma / serum nucleic acid verification.Result:Finally,several potential exosome miRNA biomarkers related to heart failure within six months after STEMI PCI were screened out: hsa-miR-4478,hsa-miR-15b-5p and hsa-miR-519d-3p Through functional annotation,we found that the node genes we are studying are highly enriched in aspects of cellular immunity,proliferation,lipid metabolism and viral infection.Conclusions:Through functional annotation,we found that the exosome miRNA we are studying may be involved in biological activities such as cellular immunity,proliferation,lipid metabolism and viral infection.We know that when myocardial injury occurs,the release of cardiac proteins induces a strong antibody-mediated immune response,which eventually becomes one of the important links leading to heart failure(HF).Paracrine mediators may be very significant,andwe will further study the biological characteristics of related exosome miRNAs.To prepare for the biomarker more effective than NT-proBNP...
