| Research background and purpose:Thyroid cancer is the most common malignant tumor in the endocrine system,its incidence increased year by year,predominantly surgical comprehensive treatment is the most effective treatment method and the traditional open thyroid cancer radical prostatectomy(open thyroidectomy,OT)will leave obvious scar in the neck with the progress of technology of cavity mirrors and the natural channel endoscopic surgery(natural orifice Transluminal eldoscopicSurgery,the concept of the NOTES),created by oral vestibular cavity mirror thyroid surgery(transoral endoscopic Thyroidectomy,TOET).The purpose:A comparative study was conducted on the clinical effects of early differentiated thyroid cancer treated by oral vestibular approach and traditional open approach combined with radical treatment of thyroid cancer and central lymph node dissection to explore the saf-ety,feasibility and effectiveness of endoscopic surgery via oral vestibular approach in the treatment of early differentiated thyroid cancer.Materials and Methods:The clinical data of 120 patients with differentiated thyroid cancer admitted to the thyroid group of general surgery department of the second affiliated hospital of chengdu medical college from January 2019 to December 2019 in the thyroid group of the general surgery department of the 416 hospital of the nuclear industry of chengdu medical college were retrospectively collected.In the open surgery group,there were 12 males and 50 females,aged 22-55 years,with a mean of 39.46±8.58 years.In the oral surgery group,there were 3 males and 55 females,aged 17-56 years,with a mean of 32.10±8.62 years.The operative time,number of lymph nodes dissected,intraoperative blood loss,postoperative drainage,postoperative pain score,patient satisfaction and surgical complications of the two operations were compared.There was no difference in operation time between the oral surgery group and the traditional open group(P>,0.05).The number of intraoperative lymph node dissection was greater than that of the open surgery group,the amount of blood loss was less,the postoperative pain score was lower than that of the open surgery group,the postoperative complications were less than that of the open surgery group,and the patient satisfaction was higher than that of the open surgery group(P<0.05),the difference was statistically significant.Result:For patients with early differentiated thyroid cancer,endoscopic surgery through the vestibular approach has the same effect as open surgery,with less blood loss,fewer postoperative complications,no obvious scar,good prognosis and high patient satisfaction,which is worthy of promotion. |
