Sub-Saharan Africa currently remains the region with the highest under five, with one in 12 children dying before his or her fifth birthday. If the actual trends continue,37 million children in sub-Saharan Africa would die in the next 15 years You et al.(2015).For this study, the extended Cox model was used to examine the risk factors of under five child mortality and check the proportional hazard assumption of the model. The percentage of distribution in the covariates have a huge difference from region to region. The risk factors that influence on under five child mortality are region, mother education level, religion, family size, age at 1st birth, mother occupation, breastfeeding, marital status, birth order, child is twin, sex of child, total children ever born, currently pregnant, smokes cigarette, birth interval, cooking fuel and wealth index statistically significant at P-Value< 0.05.Based on the findings of the study, policies and programs aimed at addressing regional variations. To achieve this, while under-five mortality reduction measures which have worked to some extent in the Affar,Amhara,Benshangul-Gumuz,SNNP and Harai must be strengthened to achieve more results in the region, these measures could be extrapolated and applied in the remaining regions of the country.
