Objective: Male vascular erectile dysfunction is one of the common causes of male infertility.Up to now,ultrasonography is the main imaging screening method for clinical improvement diagnosis.Color Doppler ultrasound can detect penile arteriovenous.But the hemodynamic condition is sometimes difficult to display for small blood vessels due to limitations in blood flow sensitivity.Superfine blood flow imaging(SMI)can retain the most accurate low-speed blood flow signal.This technical software in ultrasound equipment can display tiny blood vessels with extremely low speed,and its effect is even as good as contrast-enhanced ultrasound.It is a non-invasive,convenient and reusable ultrasound display technology.At present,there are few data on the use of SMI technology to evaluate the hemodynamics of corpus cavernosum.Therefore,the purpose of this study is to investigate the advantages and value of ultrastructural flow imaging(SMI)in the imaging and diagnosis of vascular penile erectile dysfunction(erectile dysfunction,ED),in the hope of bringing it to clinical diagnosis and treatment,and eventually be promoted and applied.Methods: 1.We selected 72 patients who were diagnosed as vascular ED(erectile dysfunction)after ultrasonography examination and another 25 healthy men as the healthy control group.SMI technique was used to detect the above-mentioned enrolled people.The blood flow spectrum of the deep arteries of the corpus cavernosum was collected,and the spectral parameters including peak systolic velocity(PSV),end diastolic velocity(EDV),and resistance index(RI)were recorded.Analysis and comparison of spectral parameters of three types of vascular ED patients and the healthy controls were performed.2.SMI technique and conventional color Doppler flow imaging(CDFI)were used to examine the 72 patients with vascular ED respectively.The 72 cases of corpus cavernosum blood flow were divided into four grades,and the differences of the two techniques in blood vessel grading were compared.3.The time required to obtain satisfactory blood flow spectrum and obtain blood flow parameters for the 72 patients by using the two techniques were recorded respectively,and were then compared.The superiority of these two technologies in the imaging process was analyzed and summarized.Results: 1.Of the 72 cases of vascular ED,19 were arterial ED,18 were venous ED,and 35 were mixed ED.The three groups were compared with the control group.The PSV values measured by SMI technique in the arterial ED group and the mixed ED group were smaller than the diagnostic criteria,and were significantly smaller than the control group,and the difference was statistically significant(P<0.01).The EDV values measured in the venous ED group and the mixed ED group were larger than the diagnostic criteria,and the RI was smaller than the diagnostic criteria.Compared with the control group,the difference was statistically significant(P<0.01).2.Imaging of CDFI and SMI techniques in the corpus cavernosum arteries.The blood flow images obtained by the two examination techniques of 72 patients with vascular ED were graded into 4 grades.The blood flow detected by CDFI technology was from low to high according to the grading standard: 26 cases,32 cases,10 cases,4 cases;the blood flow detected by SMI technique was from low to high according to the grading standard: 3 cases,6 cases,43 cases,20 cases.Analysis of blood flow grading results obtained by the two techniques.The results showed that the blood flow display effect of SMI technique was significantly better than that of CDFI technique,and the difference was statistically significant(P<0.01).The blood flow displayed by SMI technology is mainly grade 3 and grade 4,accounting for 87.5% of the total [(43+20)/72],while the CDFI technique shows blood flow of grade 1 and grade 2,accounting for the total number.80.6% [(26+32)/72],SMI showed a better effect on the cavernous artery of the penis than the CDFI technique.3.Analysis of the time required for the blood flow spectrum of the corpus cavernosum artery.Comparing the time required to obtain a satisfactory blood flow spectrum,the time required for blood flow at grade 3(1.52 ± 0.18)min,grade 4(1.21 ± 0.11)min was significantly less than grade 1(5.23 ± 0.44)min,grade 2(4.46±0.65)min,the time required for SIM technology(1.32±0.42)min was significantly less than the time required for CDFI technique(4.53±0.67)min,and the difference was statistically significant(P<0.05).Conclusion: The results of this study demonstrate that SMI technology shows better blood flow of the cavernous artery of the penis compared to conventional color Doppler(CDFI)techniques.For obtaining satisfactory spectral Doppler blood flow parameters,SMI technology can also significantly shorten the time required for examination,and has obvious superiority and application promotion value for diagnosing vascular ED.
