Objective To analyze the related factors of stress injury and explore the effect of early stress injury protection process on reducing the formation of stress injury in stroke patients.Methods 120 patients with cerebral infarction who were admitted to our hospital from May 2017 to February 2018 were selected as the research objects.Under the premise of informed consent,they were divided into observation group and control group according to the order of admission,60 cases in each group.The observation group was given routine nursing intervention,while the control group was given early stress injury protection process nursing methods.The number,depth and location of stress injury were recorded by responsible nurses.Descriptive analysis,t-test,variance analysis,Pearson correlation analysis and Logistic regression were used to analyze the general situation of high-risk patients with stress injury,the number of cases,stages,locations and the number of high-risk patients with stress injury.The patients and their family members' satisfaction with nursing and treatment were analyzed statistically.Results(1)Comparison of the incidence of stress injury in the two groups: 60 patients in the observation group,29 patients without stress injury,accounting for 48.3%,21 patients with stress injury phase 1,accounting for 35.0%,10 patients with stress injury phase 2,accounting for 16.7%.Among the 60 patients in the control group,10 patients without pressure injury occurred,accounting for 16.7%,27 cases of stress injury stage 1,accounting for 45.0%,23 cases of stress injury stage 2,accounting for 38.3%,31 cases of observation group had stress injury,accounting for 51.7%.The total incidence of stress injury in the control group was 83.3%.The total incidence of stress injury in the observation group was significantly lower than that in the control group.The difference between the two groups was significant(P<0.05).(2)The univariate analysis showed that there was no significant difference in the scores of stress injury in stroke patients of different ages and genders(P>0.05);whether there were mechanical ventilation,incontinence,sensory dysfunction,malnutrition,shock,humidity,stress,friction or shear.There were significant differences in the scores of stress injury in stroke patients(P<0.05).(3)The multiple linear regression results showed that shear force,friction,pressure,moist environment,sensory dysfunction,and malnutrition entered the regression equation.The influencing factors of stress injury in stroke patients were humid environment,pressure,friction,malnutrition,shear force and sensory dysfunction.(4)Comparison of quality of life and nursing satisfaction between the two groups: Compared with the control group,the material life,physical function,psychological function,social function and GQOLI total score of the observation group were significantly improved,and the differences between the groups were statistically significant.(P<0.05).Among the 60 patients in the observation group,40 patients were very satisfied with the nursing work,accounting for 66.7%,18 patients were satisfied with the nursing work,accounting for 30%,and 2 patients were satisfied with the nursing work,accounting for 3.3%.In the 0 cases where the work was unsatisfactory,?Among the 60 patients in the control group,29 cases were very satisfied with the nursing work,accounting for 48.3%,and 22 cases were satisfied with the nursing work.Accounted for 36.7%,9 cases were satisfied with the nursing work,accounting for15.0%,0 cases were dissatisfied with the nursing work,the total satisfaction of the patients in the observation group was better than that of the control group.High,there was a significant difference between the two groups(Z=-3.783,P=0.02).(5)Comparison of the stage and distribution of stress injury in the two groups: There were 49 cases of stage 1 and 33 stage 2 pressure injuries in 120 patients.The number of pressure injuries in sacrococcygeal region: 34,heel: 13,hip :14,lateral malleolus;8,elbow :3,shoulder: 5,knee: 2,spine: 2.The predilection site of the pressure injury was the sacrococcygeal region,followed by the hip and heel.Conclusion shear force,friction force,pressure,humid environment,sensory dysfunction and malnutrition are all the influencing factors of stress injury in stroke patients.Early intervention of stress injury protection process can reduce the incidence of stress injury in stroke patients,improve the quality of life of patients and full of nursing work.Meaning,improve the prognosis of patients.
