In recent years,with the development of liver transplantation technology,liver transplantation has become an effective way to save the lives of patients with end-stage liver disease.However,the current liver supply is extremely short.In order to expand the source of liver supply,fatty liver is gradually used as a “edge donor liver” for clinical liver transplantation.Fatty degeneration for liver transplantation has certain risks,mainly manifested as postoperative concealed or recurrent,reversible or irreversible liver function damage.At present,light-to-moderate steatosis donor liver has been recognized by most transplant centers,but severe steatosis donor liver has not been used because of its high risk of primary grafts.Therefore,how to improve the liver function after severe fatty degeneration for liver transplantation,expand the application range of marginal donor liver,and improve its safety needs further study.In 2001,Zuk et al.first isolated and cultured adipose tissue suspensions extracted from liposuction to obtain a source of new adult stem cells,namely ADSCs(adipose-derived mesenchymal stem cells).Compared with other adult stem cells,ADSCs have the advantages of easy access,large amount of material,and energy.It has the advantages of repeated material extraction and rapid cell proliferation,so it has very important research and application value.The aim of this study was to establish a model of severe fatty liver by high-fat diet and establish a model of autotransplantation in rats with severe fatty liver.Infusion of ADSCs into rats after autologous liver transplantation,To test the effect of ADSCs on severe fatty degeneration.This study is divided into three parts:In the first part,adipose-derived mesenchymal stem cells were isolated and cultured by enzymatic digestion,and the expression of surface antigen was identified by flow cytometry.The differentiation of adipose-derived mesenchymal stem cells into osteogenic and adipogenic cells was induced in vitro to obtain purity.ADSCs with high proliferation,rapid proliferation,adherence,antigen expression consistent with mesenchymal stem cells,and multi-directional differentiation ability.The second part was to determine the severe fatty degeneration of rats by high-fat diet and pathological examination.The model of severe fatty liver in rats was successfully established.The method of autologous liver transplantation was used to establish a stable model of severe fatty degeneration for liver transplantation.Summarize the main factors affecting the success of surgery,including preoperative treatment,anesthesia,microsurgery operation,choice of perfusion mode,postoperative rewarming treatment.In the third part,the rats were sacrificed by autologous liver transplantation model for severe steatosis.The rats were sacrificed at different time points.Blood tests were performed to detect liver function,total bilirubin,triglyceride index and liver tissue detection MDA(Malondialdehyde),SOD(Superoxide Dismutase)content,parallel histopathological examination.The results of the test were analyzed by analysis of variance for multiple groups and intra-group mean comparisons,and the Least-Significance Difference(LSD)method was used to compare whether ADSCs can treat liver function after liver transplantation for severe steatosis.Ischemia-reperfusion injury plays a protective role.The results showed that ADSCs had a positive effect after severe fatty degeneration for liver transplantation.It can be seen that transplantation of ADSCs can compensate liver function of damaged liver,promote bilirubin metabolism,and reduce bilirubin and triglyceride accumulation.Reduce liver ischemia-reperfusion injury.Although the specific mechanism of ADSCs for the treatment of severe fatty liver needs to be further explored,with the deepening of research,it is expected to become a powerful weapon for transforming severe steatosis liver into liver transplantation donor liver,and increase supply within a certain range.The source of the liver,expand the utilization of the liver,and bring the gospel to more patients with liver transplantation.
