Objective: To evaluate the diagnositic value of circulating miRNAs for active tuberculosis by Meta-analysis.Methods: PubMed,Web of science,Embase,CNKI,Wanfang Data,VIP,CBM were searched from inception to April,2019,to collect studies on detecting circulating miRNA as biomarkers in the diagnosis of active tuberculosis.In strict accordance with the inclusion and exclusion criteria,selected the literatures which meet the requirements,evaluated literatures quality and extracted the data.The Meta-analysis was performed by using Stata 15.0 software.Results: We retrieved 21 eligible studies involving 1142 patients with active tuberculosis,1231 controls.The pooled sensitivity(SEN),specificity(SPE),positive likelihood ratio(PLR),negative likelihood ratio(NLR)were 0.85(95%CI:0.78~0.89),0.85(95%CI:0.81~0.88),5.49(95%CI:4.21~7.15),0.18(95%CI:0.13~0.26),respectively.The pool diagnostic odds ratio(DOR)was 30.06(95%CI:16.88~53.52)and area under the ROC curve(AUC)was 0.91(95%CI:0.88~0.93).Subgroup analyses showed miRNA-155 testing yielded AUC of 0.90(95%CI:0.87~0.92)in confirming active tuberculosis.Conclusion: The detection of circulating miRNA has the advantages of small trauma,easy operation,simple and feasible,dynamic detection,and so on.miRNAs should be a significant biochemical marker for diagnosis of active tuberculosis with high sensitivity and specificity.Among them,the diagnostic value of miR-155 is outstanding.The diagnostic accuracy in pediatric patients is better than that in adults.
