Objective:To evaluate the perioperative blood coagulation function of patients in plateau area by using rapid thromboelastography, and to guide intraoperative blood component transfusion combined with the results of blood gas analysis.Method:A total of 60 patients with large intraoperative bleeding and possible intraoperative blood transfusion were selected from May to December in 2019 in Qinghai Red Cross Hospital. The patients were randomly divided into two groups: the experimental group was given blood gas analysis and r-TEG detection when entering the operating room, before and after blood transfusion. In the control group, blood transfusion was guided only according to the results of blood gas analysis, and the transfusion volume of related blood components in the two groups was recorded. After collecting the data,SPSS 26.0 software was used for statistical analysis and comparison of the difference of blood transfusion volume between the two methods during blood transfusion.Result:According to the inclusion and exclusion criteria,a total of 55 patients undergoing elective surgery at high altitude were enrolled in this study.there was no significant difference in gender composition, age, height, weight, operation time, intraoperative blood loss and infusion volume between the two groups.According to the results of r-TEG test, blood transfusion in the experimental group could significantly reduce the use of blood products, especially the use of plasma,p < 0.05,the difference was statistically significant.Conclusion:R-TEG can be used to guide the rational use of blood in patients at high altitude, reduce the infusion of blood products, especially plasma, and save blood resources.
