Object:By exploring the differences in the monocyte-high density lipoprotein ratio(MHR)between patients with coronary heart disease and healthy controls in Qinghai area,and the relationship between coronary heart disease patients with different severity of coronary artery and MHR,it provides clues for early diagnosis of coronary heart disease.Methods:By collecting 150 patients diagnosed with coronary heart disease(CHD)after coronary angiography(CAG)in the Department of Cardiology of qinghai University Affiliated Hospital from December 2018 to December 2019,they were divided into three groups according to the number of lesions in the CAG results: single branch Lesion group(41 cases),double-vessel disease group(61 cases),three-vessel disease group(48 cases),150 healthy people in the physical examination center were selected as the control group at the same time.All the subjects came to the hospital to ask general information and collect the corresponding biochemical Indicators: All patients with coronary heart disease were admitted to the hospital for emergency or elective CAG according to their condition,and the stent was implanted if necessary.The CAG results were carefully analyzed,and the score was calculated using the Gensini score.Compare and analyze whether there is a difference in MHR between the coronary heart disease group and the control group,and then explore the relationship between MHR and the severity of coronary artery disease.Results:1.When comparing the coronary heart disease group with the control group,the coronary heart disease group had a history of diabetes,hypertension,BMI,WBC,monocytes,TG,UA,and MHR all higher than the control group(P<0.05);coronary heart disease group HDL-c And LDL-c were lower than those in the control group(P<0.05),and there was no significant difference in age,sex,smoking history,HB,TC and other indicators between the two groups P<0.05)2.There were statistical differences in the levels of Mno,HDL-c,and MHR between the number of lesions(P <0.05).Further comparisons between the groups showed that the levels of Mno,HDL-c,and MHR in the single-vessel disease group and the double-vessel disease group All were statistically different(P <0.05),and the levels of Mno,HDL-c,and MHR in the single-vessel disease group and the three-vessel disease group were statistically different(P <0.05).The double-vessel disease group and the three-vessel disease group Mno,HDL-c,MHR levels were statistically different(P <0.05),there was no statistical significance in age,WBC,HB,TG,TC,LDL-c,UA between the three groups(P <0.05)..3.Correlation analysis between related indicators and clinical Gensini score: Mno was significantly positively correlated with Gensini score(r = 0.341,P <0.01),HDL was significantly negatively correlated with Gensini score(r =-0.345,P <0.05),and MHR was related to Gensini score was significantly positively correlated(r = 0.510,P <0.01)4.Logistics regression of CHD risk factors found that history of hypertension,history of diabetes,TG,age,and MHR(OR = 4.18,95% CI: 1.6-10.50,P <0.05)were all independent risk factors for CHD.5.When evaluating the predictive value of MHR to CHD by drawing the ROC curve,the area under the MHR curve(AUC)was 0.748(95% CI 0.731 ? 0.785).When the diagnostic threshold of MHR is 0.53,its diagnostic efficiency is the highest,with a sensitivity of 63.3% and a specificity of 88.3%.Conclusion:1.The MHR of patients with coronary heart disease in qinghai area is positively correlated with the clinical Gensini score of coronary heart disease,which can reflect the severity of coronary artery disease in patients with coronary heart disease and is an independent risk factor for CHD.2.When MHR> 0.53,the prediction price of coronary heart disease is higher,and MHR is simple,convenient and easy to obtain.Early detection can help clinicians identify CHD diagnosis and intervention faster.
