Objective:Collect three-dimensional reconstruction imaging by CTA technology,observation and measurement the anatomy data in medial circumflex femoral artery and femoral head related area.Include the origin of the medial circumflex femoral artery;the running form;diameter of starting section;branch way;relationship between medial femoral circumflex artery and lateral femoral circumflex artery;diameter ratio of medial femoral circumflex artery to deep femoral artery;diameter ratio of the effective length of medial femoral circumflex artery to the width of the femoral neck;number of branches spanning the line connecting the femoral head with the upper and lower intersections of the femoral neck.Looking for valuable imaging anatomy data,analyzing the relationship between the medial femoral artery and blood supply to the femoral head area,providing data support and theoretical basis for clinical practice.Materials and methods:Select patients who went to the First Hospital of Jilin University for a CTA scan of the hip from March 2018 to July 2019.According to established screening and exclusion criteria,a total of 94 cases were obtained,with 188 sides.Data analysis was performed,including 61 males and 33 females,with an average age of 49 years.Hip CTA scan using General Electric's DiscoveryTM 64-row gemstone CT machine,enhancer uses iohexol injection,scanning accuracy is 0.625 mm.Scanning accuracy digital image results are summarized to AW Volume Share 4.5 workstation supporting CT machine.It was reconstructed by workstation professional technicians.Digital 3D reconstruction of medial femoral arteries and related areas of femoral head.Further refinement based on post-processing sequences such as volume rendering and maximum intensity projection.Under the guidance of teachers and technicians in the Department of Human Anatomy,observation and data measurement are directly performed using the software provided with the workstation.Results:1.The medial femoral circumflex artery originated from the deep femoral artery in 172 cases(91.5%),85 cases on the left and 87 cases on the right.The medial femoral circumflex artery originated from the femoral artery in 16cases(19.7%),9 cases on the left and 7 cases on the right.2.Walking behavior of medial femoral artery: spiral 37 cases(19.7%),20 cases on the left and 17 cases on the right;tortuous 110 cases(58.5%),52 cases on the left and 58 cases on the right;Straight 41 case(21.8%),22 cases on the left and 19 cases on the right.3.The diameter of the initial segment of the medial femoral circumflex artery is 2.25 ± 0.9mm on the left and 2.16 ± 0.9mm on the right.4.The medial circumflex femoral artery branches up and down,called ascending and descending branches.The ascending branch is in the direction of the superior and inferior obturator;the descending branch is shaped downward to supply blood to the femoral nodules and the area below it.The main trunk of the medial circumflex femoral artery extends inward and backward,called the deep branch.The deep branch continues to orbit backwards,passing from the back of the femoral neck to its posterolateral shape and forming a part of the basilar artery ring.5.Compare the order of the medial and lateral femoral arteries,the medial circumflex femoral artery first appeared in 130 cases(69.1%),with 62 cases on the left and 68 cases on the right;the lateral circumflex arteries were first issued in 46 cases(24.5%),25 in the left and 21 in the right;12cases(6.4%)were emitted from the same plane of the medial and lateral femoral arteries,with 7 on the left and 5 on the right.6.The ratio of the medial circumflex femoral artery to the deep femoral artery at the beginning was 0.48 ± 0.14 on the left and 0.42 ± 0.16 on the right.7.The ratio of the effective contour length of the medial circumflex femoral artery to the width of the femoral neck was 0.89 ± 0.08 on the left and 0.85 ± 0.09 on the right.8.The number of branches that cross the line connecting the femoral head with the upper and lower intersections of the femoral neck is 1.46 ± 0.88 on the left,with a maximum of 3 and a minimum of 1;1.82 ± 0.95 on the right,with a maximum of 4,and a minimum of 0.Conclusion:1.The medial circumflex femoral artery originates mainly from the deep femoral artery,some from the femoral artery,and other origins are rare.The travel of the medial circumflex femoral artery and its relationship with the lateral circumflex arteries are diverse.Clinical surgery and interventional treatment need special attention in the application.2.The internal depth ratio and internal neck ratio measured by CTA imaging have certain clinical value for evaluating the relationship between the medial femoral circumflex artery and the femoral head blood supply.At this stage,CTA is not yet able to show the fine branches of the medial femoral circumflex artery extending to the femoral head and femoral neck area.
