Objective To analyze the surgical effect of congenital developmental craniovertebral junction abnormalities and summarize experience.Methods Retrospective analysis of clinical symptoms,imaging data,treatment and clinical symptoms of patients with congenital developmental craniovertebral junction abnormalities in the Department of Neurosurgery,General Hospital of Ningxia Medical University from January 1,2017 to August 1,2019.The treatment effect was evaluated by calculating the improvement rate of cervical spine JOA score.Results There were 28 patients,including 7 males and 21 females.The average age was(42.25±8.67)years.One patient had Chiari malformation,24 had a Chiari malformation with syringomyelia,15 had Chiari malformations with basilar invagination,12 had Chiari malformations with basilar invagination and syringomyelia.5 cases were treated with fusion,3cases of the PFDD groups,and 20 cases of the PFDDT groups.Postoperative complications included 1 case of suboccipital effusion and 1 case of cerebellar infarction.There was a statistically significant difference in the JOA score of preoperative and postoperative follow-up(t=-4.882,P<0.05).The effect of surgical treatment was related to the surgical method(?~2=7.742,P<0.05).Comparing the improvement rate of JOA score with or without basilar invagination in the PFDDT groups,the difference was statistically significant(t=2.101,P<0.05).Conclusion1.The use of PFDDT in the treatment of patients with symptomatic of the congenital developmental craniovertebral junction abnormalities has a good effect.2.Basilar invagination is one of the factors affecting the rate of JOA improvement in the PFDDT group.
