Objective:To explore the significance of dynamic monitoring of classical biochemical indexes in the evaluation of ischemia-reperfusion injury in orthotopic liver transplantation.Methods:The levels of biochemistry markers of 21 patients during the different time of orthotopic liver transplantation were retrospectively analyzed.The markers include serum alanine aminotransferase(ALT),aspartate aminotransferase(AST),lactate dehydrogenase(LDH),serum creatinine(SCR),urea nitrogen(BUN),cholinesterase(CHE),total protein(TP),albumin(ALB),total bile acid(TBA)and total bilirubin(TBIL).The biochemistry markers levels showed regular changes and close relationship with different operational period such as preoperative,intraoperative,postoperative and 5days after orthotopic liver transplantation.Results:LDH increased rapidly during the operation with the highest level in the period of porta hepatis opening,and gradually decreased to the normal range 5 days after the operation.ALT and AST increased during the operation and reached the highest level at the end of the operation,and decreased gradually 5 days after the operation.AST was almost normal 5 days after the operation.TBIL and TBA were in a downward trend during the operation,TBA returned to normal level 5 days after the operation.BUNbegan to increase gradually in the period of porta hepatis opening,and decreased 4 days after operation comparing with the preoperative period.Scr level did not change significantly during the total operation process and always in the normal reference range.TP began to decrease before hepatectomy,ALB did not change significantly during the operation,both of them began to rise to the normal value at 2 days after the operation.CHE did not change significantly during and 5 days after operation and was always lower than normal level.Conclusion:The change trend of classical biochemical indexes in orthotopic liver transplantation is of great significance for the evaluation of ischemia-reperfusion injury and follow-up of liver function after orthotopic liver transplantation.
