Objective:The aim of this study is to evaluate the cervical vertebral maturation and the maturational stages of midpalatal suture and zygomaticomaxillary suture of adolescent patients with class III malocclusion by using cone-beam computed tomography, and to explore the relationship between the cervical vertebral maturation and the maturational stages of midpalatal suture and zygomaticomaxillary suture, and also to analyze the diagnostic performance of the cervical vertebral maturation in estimating the accurately stages of maturation of midpalatal suture and zygomaticomaxillary suture,so as to provide certain reference for the evaluation of craniofacial growth and development of patients and provide effective theoretical basis for orthodontic clinical diagnosis and treatment.Methods:128 adolescent skeletal class ? malocclusion patients were selected from Xiamen Stomatological Hospital from 2015 to 2018.CBCT images from 128 subjects(57female,71male)were imported into the Dolphin software to define the cervical vertebral maturation, the maturational stages of midpalatal suture and zygomaticomaxillary suture. The experimental data were statistically analyzed with SPSS 24.0 software. The correlation between the cervical vertebral maturation, the maturational stages of midpalatal suture and zygomaticomaxillary suture were evaluated by the Spearman correlation test.Positive likelihood ratios were calculated to identify the reliability of the cervical vertebra maturation to evaluate the maturational stages of midpalatal suture and zygomaticomaxillary suture.Results:1.Spearman correlation test was used to analyze the correlation between the cervical vertebral maturation, the maturational stages of midpalatal suture and zygomaticomaxillary suture. The results are as follows.(1)The correlation between the cervical vertebra maturation and the maturational stages of midpalatal suture:With the spearman correlation analysis, the correlation between the cervical vertebra maturation and the maturational stages of midpalatal suture are highly related in the combined group, male group and female group, and the correlation coefficients were r=0.846,r=0.855,r=0.813(P<0.01)(2)The correlation between the cervical vertebra maturation and the maturational stages of zygomaticomaxillary suture :With the spearman correlation analysis, the correlation between the cervical vertebra maturation and the maturational stages of zygomaticomaxillary suture are highly related in the combined group, male group and female group, and the correlation coefficients were r=0.846,r=0.855,r=0.813(P<0.01)2.Positive likelihood ratios were calculated to identify the reliability of the cervical vertebra maturation to evaluate the maturational stages of midpalatal suture and zygomaticomaxillary suture.(1)The reliability of the cervical vertebra maturation in identifying the maturational stages of midpalatal suture. In the combined group, the positive likelihood ratio of judgments of the stage E of midpalatal suture by CVM 5 is 11.35.In the male group, the positive likelihood ratio of judgments of the stage A of midpalatal suture by CVM 1 is 41.29;In the female group, the positive likelihood ratio of judgments of the stage D of midpalatal suture by CVM 4 is 10.71,the positive likelihood ratio of judgments of the stage E of midpalatal suture by CVM 5 is 18.80;(2)The reliability of the cervical vertebra maturation in identifying the maturational stages of zygomaticomaxillary suture. In the combined group, the positive likelihood ratio of judgments of the stage E of zygomaticomaxillary suture by CVM 5 is 10.03.In the male group, the positive likelihood ratio of judgments of the stage A of zygomaticomaxillary suture by CVM 1 is17.67;In the female group, the positive likelihood ratio of judgments of the stage D of zygomaticomaxillary suture by CVM 4 is 10.71,the positive likelihood ratio of judgments of the stage E of zygomaticomaxillary suture by CVM 5 is 18.80;Conclusion:There was a significant positive correlation between the CVM,the maturational stages of midpalatal suture, and the maturational stages of zygomaticomaxillary suture. When the maturational stages of midpalatal suture and zygomaticomaxillary suture judged by the CVM,the CVM 5 can be used to judge the stage E of midpalatal suture and zygomaticomaxillary suture in the combined group, and the CVM 1 can be used to judge the stage A of midpalatal suture and zygomaticomaxillary suture in the male group, while the CVM 4 and 5 can be used to judge the stage D and E of midpalatal suture and zygomaticomaxillary suture in the female group. In clinical orthodontic still needs to assess the patient's stages of growth and development with age and other auxiliary diagnosis method.
