Association Between Interleukin-6 Single Nucleotide Polymorphism And HBV-associated Acute-on-chronic Liver Failure

Objective: To investigate the relationship between interleukin-6(IL-6)single nucleotide polymorphism(SNP)and the risk of Hepatitis B virus(HBV)infectious disease,including the occurrence of cirrhosis and acute-on-chronic liver failure(ACLF),also the outcome of ACLF patients.Methods: This study included HBV-infected patients who were hospitalized in the First Affiliated Hospital of Fujian Medical University from July 2012 to March 2018.We performed a case-control study,it was divided into HBV-ACLF group and non-ACLF group.SNP genotyping of eight loci in IL-6 gene(rs1524107,rs1800795,rs1800797,rs2069827,rs2069830,rs2069837,rs2069840,rs2069845)was determined by the improved multi-temperature ligase detection reaction(imLDRTM)technique.Simultaneously,case data were review the 3-months followed up survival condition of the HBV-ACLF group.Normally distributed data were expressed as arithmetic means and SDs,t-test was adopted.Data with skewed distribution were expressed as interquartile range,and were measured by non-parametric test.Multivariate logistic regression analysis was used to analyze the relative risk of genetic polymorphism and HBV-ACLF as well as the relationship between IL-6 SNPs with the occurrence and prognosis of HBV-ACLF.Clinical data,laboratory results and mortality of Hepatitis B virus infectious disease were analyzed to find the associations between clinical characteristics and IL-6 gene polymorphism.Gene-gene interactions were investigated by using multifactor-dimensionality reduction(MDR)method.Result: 1?A total of 400 patients were included,with 122 of HBV-ACLF and 278 of non-ACLF.There were significant differences in TBIL,ALT,AST,WBC,N%,PT,INR,albumin,PLT,creatinine,and MELD scores between the two groups.2?The genotypes of IL-6 gene rs1800795,rs1800797,rs2069827 and rs2069830 in all patients were not found to have mutation or mutation frequency less than 1%.There was no significant difference in the genotypes of IL-6 rs1524107,rs2069837,rs2069840,rs2069845 polymorphisms between the HBV-ACLF group and non-ACLF group(P > 0.05).3?The IL-6 rs1524107 T allele did not significantly increase the risk of HBV-ACLF,as well as the IL-6 rs2069837 A allele,IL-6 rs2069840 C allele and IL-6 rs2069845 A allele(P > 0.05).In the HBV-ACLF subgroup analysis,those SNPs were not different between the survival group and death group.4?In the non-ACLF sub group,there were 139 cases in the CHB group,and 139 cases in the cirrhosis group.There were significant differences in TBIL,PT,INR,albumin,ALT,AST,WBC and PLT between the two groups.There was no significant difference in the genotype of IL-6 rs1524107,rs2069837,rs2069840,rs2069845 polymorphisms between two groups.5?Serum IL-6 were not associated with the prognosis of HBV-ACLF.Conclusion:There is no significant association between IL-6 gene SNP and the risk of HBV-ACLF and hepatitis B cirrhosis in the Chinese Han population.IL-6 single nucleotide polymorphism has no prognostic value for ACLF.
