Objective The purpose of this study was to investigate the effect of phototherapy on pretrem infants with patent ductus arteriosus and its possible mechanism.Methods Preterm infants hospitalized in FuJian Provincial Neonatal Rescue Center during April to October,2018 were selected as study subjects.Serum NO level was detected on admission and 3 days after birth,and echocardiography was performed.According to echocardiography results at 3 days after birth,patients were divided into case group(PDA group)and control group(non-PDA group).At the same time,the cumulative phototherapy time within 3 days after birth was calculated,the relationship between the cumulative phototherapy time and the case group and the control group was compared,and the relationship between the Serum NO level and the cumulative phototherapy time in the three days after birth was analyzed.All data were analyzed via SPSS19.0.Results A total of 131 pretrem infants were included in the study,including 46 cases in the case group(PDA group)and 85 cases in the control group(non-PDA group).43cases of premature infants in the case group(PDA group)received phototherapy within3 days after birth,while 79 cases in the control group showed no significant difference(?~2=0.013,P>0.05).In the case group,the cumulative phototherapy time was(29.13±13.88)hours within three days after birth,which was significantly higher than that in the control group(22.68±9.22)hours,and the difference was statistically significant(t=3.184,P<0.05).In the case group,the first NO level was(106.90±36.25)umol/L,compared with the second NO level(91.00±17.49)umol/L,the NO level was significantly decreased,and the difference was statistically significant(P<0.05).In the control group,the first NO level was(117.99±29.18)umol/L,and the second NO level(94.48±18.38)umol/L,the NO level was also significantly decreased,the difference was statistically significant(P<0.05).Logistics regression analysis showed that prolonged cumulative phototherapy time within three days after birth was an independent risk factor for PDA in premature infants(OR=1.055,95%CI:1.013-1.100,P=0.010).The difference of NO level before and after plasma(15.91±32.66)umol/l in the case group and(23.51±35.44)umol/l in the control group.There was no correlation between the cumulative phototherapy time and the difference of NO level before and after plasma(r=0.066,P=0.454).Conclusion Long cumulative phototherapy time within 3 days after birth is an independent risk factor for continuous open arterial catheter in preterm infants,but after different cumulative phototherapy time,the difference of NO between the two groups before and after the two groups has no correlation with the cumulative phototherapy time within 3 days after birth.
