Objective:1.Application of magnetic resonance diffusion kurtosis imaging(DKI)to quantitative analysis the PD group and the control group olfactory cortex(amygdala?hippocampus?gyrus rectus?Orbital gyrus),and statistical analysis difference between two groups of olfactory cortex FA? MD ? MK value,to explore the value of DKI technology for the early diagnosis of PD.2.To analyze the correlation between the MK value of olfactory cortex,olfactory and cognitive function in PD group,and to explore the mechanism of olfactory and cognitive impairment in PD.Methods: Through case-control study,using 3.0TGE magnetic resonance diffusion kurtosis imaging analysis of case group and control group olfactory related brain areas,compare olfactory cortex FA?MD?MK value significant difference between PD group and control,and analyze PD group the relationship between olfactory cortex MK value and the age?duration?H-Y staging?olfactory test?cognitive function evaluation.ROC curve was used to determine the best Boundary MK value of olfactory cortex in the diagnostic of Parkinson's disease.Results: 1.PD group compared with the control group the left amygdala MK value,the difference was statistically significant(p<0.05);PD group according to the left amygdala MK value ROC curve showed the best cutoff value was 0.597,sensitivity65%,specificity 52.2%.There was no correlation between MK value and age,course of disease,H-Y stage,olfactory test,MMSE and FAB.2.There was correlation between MK value of right gyrus rectus and MoCA.3.PD group of MoCA,FAB compare with normal values of healthy population by single sample T test,the difference was statistically significant(p<0.05);The results of MoCA,FAB evaluation were correlated with H-Y staging.Conclusion: 1.The MK value of left amygdala in patients with primary Parkinson's disease was significantly lower than in the control group,and there was no correlation with the disease course and H-Y stage,so the left amygdala DKI may be diagnosed inthe early stage of PD.2.The optimal diagnostic value of MK in the left amygdala of patients with PD was 0.597,with sensitivity of 65% and specificity of 52.2%.3.MoCA and FAB can be used for early screening of cognitive function impairment in PD,and can be used as a tool to monitor the level of cognitive impairment in PD.4.PD cognitive impairment may be related to frontal lobe damage.5.The correlation between olfactory and cognitive evaluation(MMSE?MoCA?FAB)is still controversial.
