Study On Medication Compliance Of Goserelin In Young And Middle-aged Patients With Breast Cancer

ObjectiveInvestigate the current status of goserelin medication compliance among young and middle-aged breast cancer patients,understand and analyze the influencing factors of medication compliance,and explore strategies to improve compliance.MethodsRetrospective collection of young and middle-aged breast cancer patients who used goserelin in endocrine breast surgery in our hospital from January 2015 to December 2017.Use the general data collection form to collect the patient's name,age,family history of breast cancer,marriage General information such as status,birth history,education,occupation,perceived economic conditions,type of medical insurance,type of residence,past history,purpose of medication,time of medication,adverse reactions,AJCC staging,etc.An adapted compliance questionnaire was used to investigate the status of medication compliance among young Luminal and triple-negative breast cancer patients.Logistic regression andFisher's exact test were used to analyze the relevant influencing factors of goserelin medication compliance in these patients.Results(1)Among the young and middle-aged breast cancer patients,goserelin is mainly used for the treatment of Luminal and triple negative breast cancer.A total of 211 patients with Luminal and triple negative breast cancer were included in this study.185 patients were successfully followed up with an overall follow-up success rate of 87.7%,and 107 patients with good compliance had an overall compliance rate of 57.83%.(2)In this study,174 patients with Luminal breast cancer were included,and 154 patients were successfully followed up.The follow-up success rate was 88.5%,and the compliance rate was 53.3%.Univariate analysis showed that education level(?~2=13.586,P=0.000),type of medical insurance(?~2=13.042,P=0.001),adverse reactions(?~2=7.88,P=0.005),and whether you understand the course of goserelin Time(?~2=14.251,P=0.001),the difference was statistically significant(P<0.05).Logistic regression analysis results showed that: high school and below(P=0.020,OR=0.383,95%CI=0.171~0.859),residents' medical insurance(P=0.010,OR=0.168,95%CI=0.043 ~ 0.656)and don't know Goserelin's treatment duration(P=0.005,OR=0.347,95%CI=0.166~0.726)is a risk factor for compliance.(3)A total of 37 patients with triple negative breast cancer were included,and 31 cases were successfully followed up.The follow-up success ratewas 83.8%,and the compliance rate was 80.6%.The compliance was good.Fisher's exact test results showed that education level(P = 0.022)and economic conditions(P = 0.026)had an impact on compliance,and the difference was statistically significant(P <0.05).The compliance of unmarried and unfertile patients is 100.0%,which is better than that of married(76%)and fertile(70%)patients;the compliance of patients who understand the purpose of medication(87.0%)and the duration of treatment(90.0%)is better than that of not understanding Patients with the purpose of medication(62.5%)and course of treatment(76.2%);compliance of patients with employee medical insurance(88.9%)and resident medical insurance(87.5%)is better than patients with other medical insurance types(40.0%).Conclusion(1)The overall compliance of young and middle-aged breast cancer patients using goserelin is poor,the compliance of Luminal breast cancer patients is poor,and the compliance of triple negative breast cancer patients is good.Therefore,medical staff should strengthen the supervision and management of the use of Goserelin in patients,especially the compliance of Luminal breast cancer patients.(2)There are many factors affecting the compliance of goserelin medication in young and middle-aged Luminal breast cancer patients,mainly education level,medical insurance type,and whether they understand the purpose and duration of treatment.Clinicalmedical staff should strengthen the management and follow-up of out-of-hospital medications for young and middle-aged Luminal breast cancer patients.They should pay attention to factors such as the education level of the patients and the type of medical insurance,and formulate corresponding health education strategies according to the individual conditions of the patients.(3)The main influencing factors of medication compliance of goserelin in young and middle-aged patients with triple-negative breast cancer are fertility status,type of medical insurance,and whether they understand the purpose and time of medication.Therefore,while paying attention to factors such as the education level of the patient and the type of medical insurance,the medical staff should fully understand the patient's fertility needs and wishes,monitor their medication use,and improve the patient's medication compliance.
