Objective: To explore the correlation between ABO blood type and acute ischemic stroke, study the influence of blood type factors on acute ischemic stroke, and propose new insights and approaches for the prevention and treatment of acute ischemic stroke. Methods: A total of 146 patients with acute ischemic stroke hospitalized in the Department of Neurology of Shaanxi Provincial People's Hospital from January 2019 to December 2019 were collected as the observation group, and 125 healthy medical examinees who were outpatients or admitted to the same period as the control group were collected.Patients in the observation group were categorized using TOAST etiology typing. The national institutes of health stroke scale score was used to assess the severity of the patients in the observation group on the day of admission. All subjects included in the study were excluded: age < 18;the nationality is non-han;coronary atherosclerotic heart disease;venous thrombosis and other arterial thrombotic diseases;severe cardiac and hepatic dysfunction;severe inflammatory diseases and immune diseases;severe thyroid dysfunction;diseases related to the blood system;benign and malignant tumors;statins, ezetimibe, fibrates and other drugs were taken orally within 1 month before admission;incomplete imaging data or laboratory results.General and basic information,ABO blood types and lipid levels of all selected participants were collected. The correlation between ABO blood group and acute ischemic stroke and its mechanism were analyzed by statistical processing of the data. Results: 1.Compared with O blood type,non-O blood type,B blood type and AB blood type had a higher risk of acute ischemic stroke, which was significantly correlated with the incidence of acute ischemic stroke. The OR values were 2.135(95%CI:1.257-3.625,P=0.005),2.364(95%CI:1.265-4.415,P=0.007)and 2.512(95%CI:1.058-5.966,P=0.034),respectively.After multivariate logistic regression analysis, the results still showed that the risk of acute ischemic stroke in people with blood type B was higher than that in people with blood type O,OR was 2.379(95%CI:1.254-4.514,P=0.008);2.In the correlation analysis between ABO blood type and LAA group, compared with O blood type, the risk of acute ischemic stroke was increased in non-O blood group,A blood group,B blood group and AB blood group, with OR values of 2.761(95%CI:1.395-5.465,P=0.003),2.622(95%CI:1.197-5.744,P=0.015),2.571(95%CI:1.159-5.703,P=0.019)and 3.740(95%CI:1.360-10.288,P=0.009);3.There were no statistically significant differences in baseline NIHSS scores among acute ischemic stroke patients with different ABO blood type(F=0.495,P=0.739);4.Plasma total cholesterol(TC)level in patients with non-O blood type,B blood type and AB blood type was higher than that in patients with O blood type(t=2.181,P=0.031;T = 2.201,P = 0.030;T = 2.783,P = 0.008);Patients with blood type B had higher levels of LDL cholesterol(LDL-C)than those with blood type O(t=2.280,P=0.025).Conclusions: 1.Compared with those of blood type O,non-O blood type,B blood type, and AB blood type have an increased risk of acute ischemic stroke.Blood type B can be used as a risk factor for acute ischemic stroke;2.Compared with other types of ischemic stroke,ABO blood type may have a stronger correlation with LAA stroke;3.ABO blood type has no significant correlation with the degree of neurological deficit in acute ischemic stroke;4.The total plasma cholesterol level of patients with non-O blood type,B blood type and AB blood type is higher than that of blood type O patients, and blood type B patients have higher LDL cholesterol levels than those of blood type O.
