Relationship Between Klotho Protein And Frank Sign In Patients With Chronic Kidney Disease

Abstract/Summary:
Background and purpose:Klotho protein is a peptide substance related to the aging phenotype.The expression of Klotho protein in chronic kidney disease(CKD)patients continues to decrease;Frank sign is the earlobe fold sign,which is related to coronary heart disease,aging and other factors.This study was conducted to explore the relationship between the protein level of serum Klotho and Frank sign in patients with CKD,to offer help for the diagnosis and treatment of patients with CKD.Method:234 patients with CKD3-5 or normal renal function who were hospitalized in the Department of Nephrology and the First Department of Cardiology of Shaanxi Provincial People’s Hospital from May 2018 to March 2019 were selected,including 149 males and 85 females.All subjects in this study were divided into 99 cases of CKD group and 135 cases of the non-CKD group,of which are 136 cases of Frank sign negative group,98 cases of Frank sign positive group.General clinical data of all enrolled cases was collected such as gender,age,medical history,and serum creatinine,urea,and blood lipids.Serum Klotho protein levels were detected by enzyme-linked immunolabeling(ELISA).The results of continuous numerical variables(by the normal distribution)are expressed as mean ± standard deviation,compared by t-test,and the results of categorical variables are expressed as percentages,compared by the chi-square test,and P <0.05 is considered statistically significant.The Logistic regression was used to exclude the influence of confounding factors,analyzing the correlation between CKD and Frank sign,CKD and Klotho protein,Frank sign and Klotho protein.Spearman correlation coefficient compared the correlation between Klotho protein and Frank sign in CKD group and the non-CKD group.Result:The positive rate of Frank sign in the CKD group was 53.5%,and the positive rate of Frank sign in the non-CKD group was 33.3%.The difference in the positive rate of Frank sign between the two groups was statistically significant(P <0.05);the logistic regression model showed the significant relevance of CKD and Frank after excluding confounding factors.OR value was 6.59,95CI% was 3.06-14.20,P <0.05.The Klotho protein concentration in the CKD group was 11.38±2.37ng/ml,and the Klotho protein concentration in the non-CKD group was 509.23±303.46ng/ml.The difference in Klotho protein concentration between the two groups was statistically significant(P <0.05);the logistic regression model excluded confounding factors and the result indicated that there was a significant correlation between CKD and Klotho protein concentration,which OR value was 0.73,95% CI was 0.20-0.99,P <0.05.The Klotho protein concentration in the Frank sign negative group was 388.49 ± 364.50ng/ml,and the Klotho protein concentration in the Frank sign positive group was 173.86±247.40 ng / ml.The difference in Klotho protein concentration between the two groups was statistically significant(P<0.05);the logistic regression model showed a significant correlation between Frank sign and Klotho protein after excluding confounding factors,which OR value was 0.86,95% CI was 0.25-0.91,P <0.05.Klotho protein had a significantly strong correlation with the Frank sign in the CKD group(P<0.05,∣r∣=0.84,95%CI was-0.25--0.63),and in the non-CKD group Klotho protein had a significant lower correlation(P<0.05,∣r∣=0.33,95%CI was-0.49--0.16).Conclusion:(1)Serum Klotho protein level is correlated with Frank sign.(2)The correlation between serum Klotho protein level and Frank sign is more significant in the CKD3-5 patients than the counterpart in non-CKD population.(3)CKD is correlated with Frank’s sign.(4)CKD is correlated with serum Klotho protein levels.
