Effects Of Repetitive Transcranial Magnetic Stimulation Combined With Exercise Therapy On Gross Motor Function In Children With Spastic Cerebral Palsy

Objective: Spastic cerebral palsy is the most important cause of physical disability in children,and there is no effective treatment.At present the main way for the treatment of cerebral palsy at home and abroad such as medicine,surgery and rehabilitation,the main strategy for treating cerebral palsy is found early treatment,early and timely long-term rehabilitation training as auxiliary,through clinical research found that in the current level of the medical treatment such as medicaments,operation method can only be used for rehabilitation training to lay the foundation and doesn’t replace the rehabilitation training,therefore,this article will focus on research using different frequency rTMS(repetitive Transcranial Magnetic Stimulation,and combined with the effect of exercise therapy on spastic cerebral palsy,to analyze the efficacy of gross motor function in children with spastic cerebral palsy and further analyze its feasibility,so as to provide a more appropriate and effective rehabilitation treatment for children with spastic cerebral palsy.Methods: 45 children with cerebral palsy who received cerebral palsy rehabilitation treatment in the pediatric rehabilitation training room of Hebei General Hospital from July 2017 to January 2018 were selected as the study subjects.By means of numerical table method,they were randomly divided into the control group,the high-frequency stimulation group and the low-frequency stimulation group with 15 cases each.Control group male children in 8 cases,female were 7 cases,children aged from 2 to 7 years old,the mean age for(4.51 + 0.62)years of age,course of the disease in children with 2 to 5 years,the average(3.07-0.96)years,use of this group of children with GMFCS(Gross Motor Function Classification System,cerebral palsy Gross Motor Function Classification System)to assess grading results as follows: 8 children with level I,4 children with level II,and 3 children with level III.In the high-frequency stimulation group,11 male children and 4 female children were aged between 3 and 7 years old with an average age of(4.46±0.61)years.The disease duration of the children was 2-6 years with an average of(3.40±1.18)years,use of this group of children with GMFCS to assess grading results as follows: 7 children with level I,4 children with level II,and 4 children with level III.In the low-frequency stimulation group,there were 10 male children and 5 female children,aged between 3 and 6 years old,with an average age of(4.25±0.66)years.The disease duration of the children was 2-5 years,with an average of(3.25±1.09)years,use of this group of children with GMFCS to assess grading results as follows: 6 children with level I,5 children with level II,and 4 children with level III.Three groups of children were treated with rTMS of different frequencies(pseudo-stimulation/5Hz/1Hz)on the basis of routine rehabilitation training.The course of treatment was 12 weeks,and the children were scored after 8 and 12 weeks.During the whole study,subjects were cared for and treated by the same rehabilitation therapist to ensure consistency of treatment methods.GMFM(Gross Motor Function Measure)and MAS(Modified Ashworth Scale)were used to evaluate the gross motor function of children in the three groups,as well as the quality scores of triceps surae of children in the three groups.Results:(1)The scores of the three groups before and after 8 weeks treatment had significant changes,and the data of the three groups were statistically significant(P< 0.05).(2)Compared with the control group,GMFM scores in the low-frequency stimulation group and the high-frequency stimulation group were significantly improved in the D-energy area,E-energy area(P<0.05).(3)MAS scores of triceps surae in the low frequency stimulation group and the high frequency stimulation group were significantly lower than those in the control group(P<0.05).(4)There was no significant difference in GMFM or MAS between the low-frequency stimulation group and the high-frequency stimulation group(P>0.05).(5)The D-energy area and E-energy area of GMFM in the low-frequency stimulation group and the high-frequency stimulation group after 8 weeks and 12 weeks of treatment were not significantly different from the GMFM scores in D-energy area and E-energy,or MAS scores of triceps triceps surae(P>0.05).Conclusion:(1)Taking exercise therapy of routine rehabilitation training,and combining the rTMS,body function in children with spastic type cerebral palsy of gross motor function,has obvious curative effect.(2)The appropriate course of the treatment is 8 weeks.
