Objective: To evaluate the clinical value of the modified pre-and retro-auriculary incision(also known as v-shaped incision,v-shaped cosmetic incision)in parotid benign tumors resection by comparing it with traditional "S" incision in operation,post-operation effect, patient satisfaction and other aspects. Methods: 51 patients with benign parotid tumors who were treated in the First Affiliated Hospital of Jinan University from March 2018 to November 2018 were randomly divided into two groups. In the experimental group,23 patients were treated with the modified pre-and retro-auriculary incision, while in the control group,28 patients were treated with traditional "S" incision. The facial nerve was dissected by anterograde method, the parotid masseter fascia, greater auricular nerve and the parotid duct were preserved, and the mass and its surrounding normal glands of about0.5cm were excised together in both groups.Data were collected and followed up after operation, recording the operation time, intraoperative bleeding volume, postoperative drainage volume and the length of scar. Objective evaluation of facial nerve injury and auricular nerve injury was conducted on the seventh day and in three months after the surgery, and the patients were asked to complete the parotid gland surgery questionnaire. In three months after the surgery, the doctors filled out the SCAR scale. Results: There was no significant difference in bleeding volume and negative pressure drainage volume between the modified pre-and retro-auriculary incision group and the traditional "S" incision group. There was significant difference in operation time between the two groups(P < 0.05),the the modified pre-and retro-auriculary incision needed longer operation time. There was no significant difference in subjective and objective evaluation of facial nerve injury and greater auricular nerve injury between the two groups. There was no statistical difference between the two groups in SCAR scale evaluation. However,in the modified pre-and retro-auriculary incision group, the scar length was shorter and patients' satisfaction with scar was higher. Conclusion: Compared with the traditional S-shaped incision, the modified pre-and retro-auriculary incision reduces postoperatively visible scar and significantly improves the aesthetic appearance of the operation on the basis of guaranteeing the surgical effect and not increasing the surgical complications. The patients are highly satisfied with the incision, which is worthy of being popularized in clinical practice.
