| Objective: By observing and comparing the clinical effects and divergent assessments between the method of “Song's orthopedic”and the method of “surgical treatment” for AO / OTA-C1 type distal radius fracture(DRF),to evaluate the clinical effect and advantages in the AO / OTA-C1 type distal radius fracture and systematically summarize the experience and inherit the academic thought of “Song's orthopedic”,Methods: those 51 cases that were in the sympom of AO/OTA-C1 type fracture and collected from the orthopedy department of the Affiliated Hospital of Gansu University of Chinese Medicine,of their own accord,would be divided into two groups: the “surgical treatment”group and the “Song's orthopedic”group.The “surgical treatment”group included 29 cases.The “Song's orthopedic”group included 22 cases.The two groups of patients were followed up for half a year to observe the pain score of Visual analog score(VAS),swelling degree,radiography improvement,fracture healing time,active movement range and function score of wris tjoint,clinical cost.Above rusults couid be construe by SPSS 22.0.Results:(1)VAS: on the 3 days after treatment,the VAS scores of the patients in the control group and the treatment group were significantly lower than those before treatment(P < 0.05),and the VAS scores in the operation group were significantly higher than those in the song's orthopedic group(P < 0.05).Compared with 3 days after treatment,the VAS scores of the two groups were statistically different(P < 0.05).The VAS score of song's orthopedic group was lower than that of operation group,but there was no significant difference between the two groups(P > 0.05).(2)The degree of swelling: 3 days after the treatment,the swelling degree of the operation group was significantly improved(P < 0.05),and the swelling degree of the song's orthopedic treatment group was increased,there was no statistical difference compared with that before the treatment(P > 0.05).The swelling degree of the operation group was lower than that of the song's orthopedic treatment(P < 0.05).One week after treatment,the swelling degree of the wrist in the two groups was significantly lower than that before treatment and three days after treatment(P < 0.05).At this time,the swelling degree of song's bone setting therapy was lower than that of surgery(P < 0.05).(3)Comparison of radiography: compared with that before treatment,the anatomical structures of the control group and the treatment group,such as ulnar deflection angle,palmar inclination angle,radial styloid process height,were recovered one week after treatment(P < 0.05),but the operation treatment was better than the song's osteotomy(P < 0.05).At the last follow-up,there were still significant differences in the anatomical structures of ulnar declination,palmar declination,ulnar declination,palmar declination and height of radial styloid process between the two groups compared with that before treatment(P < 0.05);compared with that one week after treatment,the palmar declination,ulnar declination and height of radial styloid process between the two groups decreased,but there was no statistical difference(P > 0.05).(4)Time of fracture healing: the time of fracture healing in Song's orthopedic group was shorter than that in the operation group,and there was statistical significance between the two groups(P < 0.05).(5)Gartland werley score: at the last follow-up,the excellent and good rate of song's orthopedic group was 72.4%,the excellent and good rate of operation group was 72.8%,and the comparative effect of wrist function between the two groups was similar(P > 0.05).(6)Active range of wrist movement: in the last follow-up,the active range of wrist movement(wrist extension,wrist flexion,wrist radial deviation,wrist ulnar deviation and pronation supination)in Song's bone setting group was smaller than that in the operation group,but there was no significant difference between the two groups(P > 0.05).(7)Economic evaluation: at the last follow-up,the cost of song's bone setting therapy was significantly lower than that of surgery(P < 0.05).(8)Complications: the incidence of complications was 13.79% in Song's osteotomy group and 18.18% in operation group.There was no significant difference between the two groups(P > 0.05).Conclusions:1.Both Song's orthopedic therapy and surgical treatment can effectively restore the anatomical structure of the distal radiusfor the AO / OTA-C1 DRF,but surgical treatment has obvious advantages for the recovery of anatomical structure.Compared with surgical treatment,Song's orthopedic therapy has obvious advantages in swelling,analgesia,fracture healing time and clinical cost.It is equivalent to the clinical efficacy of surgical treatment in the recovery of wrist joint function and the active range of wrist.2.The characteristics of the Song's orthopedic therapy are as follows:(1)It underlines the integration between doctors and patients,and highlight the idea of putting people first;(2)This remedy combined the traditional Chinese medicine and the orthopedic therapy,accordingly achieving the purpose of rehabilitation. |
