Objective: Persistent atrial fibrillation and paroxysmal atrial fibrillation have different atrial fibrillation burden.The purpose of this article is to explore the correlation of risk factors for the prognosis of elderly patients with different atrial fibrillation burden and heart failure.Methods: A total of 164 elderly patients with atrial fibrillation and heart failure were screened from October 2017 to January 2019 in the Geriatric department and Cardiology department of the First Affiliated Hospital of Chongqing Medical University.Collect their clinical data,including the general condition of the patient,the type of AF,the NYHA classification,the history of other diseases,hemoglobin,total bilirubin,direct bilirubin,creatinine,eGFR,triglyceride,total cholesterol,NT-proBNP,D-dimer,left atrium diameter,right atrium diameter,ejection fraction,etc.Patients with multiple admissions surveyed for their first admission and follow-up every 4 months after discharge.And define rehospitalization,all-cause death as the end event.Statistical analysis was performed by using SPSS24.0 statistical software package.Measurement data is expressed by?X±s,and count data is expressed by absolute value and percentage..Univariate analysis was performed using t-test,rank sum test,or chi-square test.Multivariate logistic regression analysis affected the independent risk factors for the prognosis of patients with clinical events.All analyses used bilateral 95% confidence interval(CI)as the statistical standard.P <0.05 was considered statistically significant.Results: The average follow-up of patients included in this study was13.32±3.25 months,and the total incidence of clinical events was 98 cases(59.7%).In elderly patients with persistent atrial fibrillation and heart failure,increased age(OR = 1.069,95% CI 1.008-1.135,P = 0.026)and increased left atrial diameter(OR = 1.168,95% CI 1.062-1.285,P = 0.001)are independent risk factors for clinical events.In elderly patients with paroxysmal atrial fibrillation and heart failure,coronary heart disease(OR= 5.031,95% CI 1.063-23.805,P = 0.042)and increased NT-pro BNP(OR= 1.001,95% CI 1.000-1.002,P = 0.030)are independent risk factors for clinical events.The above differences are statistically significant(P <0.05).Conclusion: The results of this study found that in elderly patients with atrial fibrillation and heart failure,different atrial fibrillation burden have different risk factors that affect their prognosis.Elderly patients with persistent atrial fibrillation and heart failure who have a higher atrial fibrillation burden,increased age and left atrium diameter are independent risk factors for the risk of clinical events.Coronary heart disease and increased NT-pro BNP are independent risk factors for the risk of clinical events in elderly patients with paroxysmal atrial fibrillation and heart failure.
