BackgroundThere are many patients with viral hepatitis in China,and chronic hepatitis B is the most common.For patients with hepatitis B cirrhosis and portal hypertension,splenectomy is the main surgical method for the treatment of cirrhosis with portal hypertension.But at the same time,postoperative complications are very common,and some complications may be fatal.ObjectiveThis study mainly evaluated the risk of postoperative complications through the comprehensive complication index(CCI)after splenectomy in patients with liver cirrhosis,and identified risk factors for patients with a high postoperative CCI score.MethodsA retrospective study was conducted on 67 patients with viral hepatitis-related cirrhosis and complications,who underwent selective splenectomy in our hospital from January 2009 to January 2016.The main evaluation method is the CCI score.A CCI score of 30 is considered a high CCI score.Through group comparison and statistical analysis,the risk factors are established,and further analysis is conducted on the basis of again to determine independent risk factors.ResultsThe median CCI score of this group of patients was 21.7(range: 7.7-61.1),of which 21 patients(31.3%)had a CCI score higher than 30.Univariate and multivariate analysis showed that the risk factors associated with high CCI score were gender,hypertension,antiviral therapy before surgery,platelet levels before surgery,and surgical methods,among which hypertension and antiviral therapy before surgery were related Independent risk factors.ConclusionTThe CCI score is a useful grading system for assessing the occurrence of complications after splenectomy in patients with liver cirrhosis.Hypertension is an independent risk factor for spleen excision surgery,it is recommended to control blood pressure before surgery,and postpone surgery if necessary.Surgical methods and receiving antiviral treatment before surgery may affect the outcome of splenectomy,but need more sample support.
