Objective: Chronic obstructive pulmonary disease(COPD)is one of the most common chronic diseases in the world,which is a grave threat to human health.Acute exacerbation of chronic obstructive pulmonary disease(AECOPD)is the main reason for patients to seek medical treatment,which will not only lead to a rapid decline of lung function,but also affect other organs,bringing a huge burden to individuals,families,and health system.It is of great significance for patients to evaluate the severity and predict the prognosis of patients with acute exacerbation of COPD in a simple and convenient way.Airway inflammation is considered to be an important factor in the pathogenesis of COPD.Studies have shown that progranulin(PGRN)is highly expressed in airway epithelium cells and it participates in the regulation of inflammation in a variety of diseases.The purpose of this study is to investigate the changes of serum PGRN levels before and after treatment in patients with AECOPD and evaluate its clinical significance.Methods: A total of 97 AECOPD patients were included in this studyas observation group and 32 healthy adults were enrolled as the control group.We measured the levels of serum PGRN in the control group and the patients with AECOPD on day 1 and day 7 after their admission and record the clinical data of the observation group,then analyze the correlation with serum PGRN.Results: The serum PGRN level of AECOPD patients before and after treatment was significantly higher than that of the control group(92.36±8.25ng/ml).Compared with day 1 after admission(126.64± 13.73ng/ml),the serum PGRN level of AECOPD patients on day 7(101.16±10.76ng/ml)were significantly decreased and the difference was statistically significant(P<0.05).Correlation analysis indicated that serum PGRN level of AECOPD patients before treatment was negatively correlated with the percentage of forced expiratory volume in the first second(FEV1/pred)(r=-0.635,P <0.001),and positively correlated with CAT score(r =0.664,P <0.001),C-reactive protein(CRP)(r = 0.871,P <0.001),neutrophil count(NEUT)(r =0.786,P =0.005)and smoking status(r =0.576,P <0.001),and the serum PGRN level of AECOPD patients after treatment was still positively correlated with the CAT score(r = 0.084,P =0.002)and smoking status(r =0.598,P <0.001).Finally,through multivariate linear regression analysis,statistical results demonstrated FEV1/pred,CRP and CAT score were independent parameters associated with serum PGRN.Conclusion: PGRN may be involved in the pathogenesis of AECOPDby interfering with inflammatory response,and its dynamic changes has clinical significance for the evaluation of disease severity and prognosis.
