Study Objective:To find out whether ABO Blood Group is associated with increased intraoperative blood loss in cervical surgery.,and to explore the effect and clinical significance of ABO blood group on perioperative bleeding.Methods:To collect patients who underwent anterior cervical fusion surgery in our hospital between August 2013 and August 2019.Types of anterior cervical fusion surgery included Anterior cervical corpectomy decompression and fusion(ACCF)or Anterior cervical discectomy and fusion(ACDF).According to the difference of the ABO blood group,the patients were divided into O blood group and non-O blood group.According to the difference of patient's operation,the patients were divided into two subgroups(ACCF group and ACDF group).Intraoperative blood loss was assessed by EBL(external blood loss),Drop-Hct(hematocrit)and Drop-Hb(hemoglobin).This study collected indicators such as ABO blood group,Preop Hct,Preop Hb,Postop Hct,Postop Hb,history ofhypertension and etiology.Statistical analysis was performed to compare the difference between blood group O and blood group non-O.Results:1.126 patients were included in the study.There were 22 patients in the O blood group and 104 patients in the non-O blood group.There were no significant differences in age and gender between the two groups.In the blood group O,the average value of EBL in patients was 80.0 ± 53.1 ml.The average value of EBL in patients with blood group non-O was 120 ±114 ml.There was no statistically significant difference(p = 0.263).There was no significant correlation between ABO blood type and EBL.2.The Drop-Hb value of patients in blood group O is 15(11.9-20.2)g/ L,and the Drop-Hb value of patients in blood group non-O is 8.5(6.5-11.0)g / L.The value of Drop is higher,and the difference is statistically significant(p <0.001).The Drop-Hct value of patients in type O group was 4.8(3.3-6.9)%,the drop value of patients in blood group non-O group was 2.2(1.6-2.8),and the drop value of patients with type O blood was higher,and the difference was statistically significant(p <0.001).3.There were 54 patients in the ACCF subgroup(8 patients with blood group-O and 46 patients with blood group non-O).The Drop-Hb value of patients with blood group O was 17.5(14-24)g / L.In the blood group non-O,the Drop-Hb value was 11.0(7.0-11.1)g / L,and the Drop-Hb value of the blood group O was higher,and the difference wasstatistically significant(p = 0.003).The Drop-Hct value of patients in blood group O was 5.2(4.2-7.4)%,the Drop-Hct of patients in blood group non-O was 2.3(1.8-2.8),and the Drop-Hct value was higher in patients with blood group O,and the difference was statistically significant Significance(p = 0.002).4.There were 72 patients in the ACDF subgroup(58 patients with O-type blood and 14 patients with non-O-type blood).The Drop-Hb value of patients in the blood group O was 12.0(9.0-24.0)g / L.In the blood group non-O,the Drop-Hb value was 7.0(2.1-9.0)g / L.The Drop-Hb value of the blood group O was higher,and the difference was statistically significant(p = 0.002).The Drop-Hct value of patients in blood group was3.3(2.6-7.3)%,the Drop-Hct value of patients in blood group non-O was1.9(0.7-3.2),and the Drop-Hct value of patients with blood group O was higher,and the difference was statistically significant Significance(p =0.004).Conclusion:1.In the anterior cervical fusion surgery,there is no obvious correlation between ABO blood type and EBL.This result may be caused by the inaccuracy of the visual estimation method.2.In the anterior cervical fusion surgery,patients with blood group O have higher Drop-Hb and Drop-Hct.In the ACDF group and the ACCF group,Drop-Hb and Drop-Hct in the patients with blood group O are stillhigher,indicating that patients with blood group O have a potential risk factor for increased intraoperative blood loss in anterior cervical fusion surgery.For patients with blood group O,we should pay more attention to hemostasis during the operation,and use hemostatic drugs when necessary to reduce the possibility of related complications.
