Objectives:To observe the effect of the active cycle of breathing technique combined with 5A model in patients undergoing lung cancer surgery.To observe the effect of the active cycle of breathing technique in patients undergoing lung cancer surgery.Methods:This study was a single-center,prospective randomized controlled trial.102 patients with lung cancer surgery were randomly assigned to the active cycle of breathing technique combined with 5A model group,the active cycle of breathing technique group and the control group.Patients in the control group received usual care.Patients in the active cycle of breathing technique combined with 5A model group received active cycle of breathing technique combined with 5A model intervention based on usual care.Patients in the active cycle of breathing technique group received only active cycle of breathing technique based on usual care.Outcome measures were dyspnea,exercise self-efficacy,6-minute walking distance,anxiety,depression,postoperative exercise compliance,and pulmonary complications.The evaluation times were at admission day,1 day before surgery,1 day after surgery,3 days after surgery,and discharge day.Statistical software SPSS24.0 was used for data analysis.Results:1.A comparative analysis of the demographic characteristics of the three groups of patients found that there was no statistically significant difference in age,gender,current smoking status,current drinking status,number of patients with chronic obstructive pulmonary disease,body mass index,length of hospital stay,and pulmonary function(P>0.05).2.There was no statistically significant difference in dyspnea,exercise self-efficacy,6-minute walking distance,anxiety and depression at admission between the active cycle of breathing technique combined with 5A model group,the active cycle of breathing technique group and the control group(P>0.05).3.Compared with the control group,the active cycle of breathing technique combined with 5A model group had no statistical difference in 6-minute walking distance and depression before surgery(P>0.05),and there were statistical differences in dyspnea(P=0.010),exercise self-efficacy(P=0.030)and anxiety(P=0.005).Compared with the control group,the active cycle of breathing technique group had no statistical difference in the exercise self-efficacy,6-minute walking distance,and depression of the patients before surgery(P>0.05),and there were statistical differences in dyspnea(P=0.037)and anxiety(P=0.046).Compared with the active cycle of breathing technique group,the active cycle of breathing technique combined with 5A model group had no statistical difference in dyspnea,exercise self-efficacy,6-minute walking distance,anxiety,and depression before surgery(P>0.05).4.Compared with the control group,the active cycle of breathing technique combined with 5A model group had no statistical difference in the 6-minute walking distance and depression on the first day after surgery(P>0.05),and there were statistical differences in dyspnea(P<0.001),exercise self-efficacy(P<0.001),anxiety(P=0.004),and postoperative exercise compliance(P<0.001).Compared with the control group,the active cycle of breathing technique group had no statistical difference in the 6-minute walking distance and depression of the patients on the first day after surgery(P>0.05),and there were statistical differences in dyspnea(P=0.012),exercise self-efficacy(P <0.001),anxiety(P=0.020),and postoperative exercise compliance(P<0.001).Compared with the active cycle of breathing technique group,the active cycle of breathing technique combined with 5A model group had no statistical difference in dyspnea,6-minute walking distance,anxiety,and depression on the first day after surgery(P> 0.05),and there were statistical differences in exercise self-efficacy(P<0.001),and postoperative exercise compliance(P<0.001).5.Compared with the control group,the active cycle of breathing technique combined with 5A model group had no statistical difference in the 6-minute walking distance between the two groups in the third day after surgery(P>0.05),and there were statistical differences in dyspnea(P<0.001),exercise self-efficacy(P<0.001),anxiety(P <0.001),depression(P=0.001),and postoperative exercise compliance(P<0.001).Compared with the control group,the active cycle of breathing technique group had no statistical difference in the 6-minute walking distance after three days of operation(P> 0.05),and there were statistical differences in dyspnea(P=0.003),exercise self-efficacy(P<0.001),anxiety(P<0.001),depression(P=0.002),and postoperative exercise compliance(P<0.001).Compared with the active cycle of breathing technique group,the active cycle of breathing technique combined with 5A model group had no statistical difference in dyspnea,anxiety,and depression after three days of operation(P>0.05),and there were statistical differences in exercise self-efficacy(P<0.001),6-minute walking distance(P=0.009),and postoperative exercise compliance(P<0.001).6.Compared with the control group,the active cycle of breathing technique combined with 5A model group showed no statistical difference in the 6-minute walking distance between the two groups of patients at discharge(P>0.05),and there were statistical differences in dyspnea(P<0.001),exercise self-efficacy(P<0.001),anxiety(P <0.001),depression(P<0.001),postoperative exercise compliance(P<0.001),and pulmonary complications(P=0.013).Compared with the control group,the active cycle of breathing technique group had no statistical difference in the 6-minute walking distance between the two groups of patients at discharge(P>0.05),and there were statistically significant differences in dyspnea(P=0.003),exercise self-efficacy(P <0.001),anxiety(P<0.001),depression(P=0.001),postoperative exercise compliance(P <0.001),and pulmonary complications(P=0.045).Compared with the active cycle of breathing technique group,the active cycle of breathing technique combined with 5A model group had no statistical difference in anxiety,depression,and pulmonary complications at discharge(P>0.05),and there were statistical differences in dyspnea(P<0.001),exercise self-efficacy(P<0.001),6-minute walking distance(P=0.011),and postoperative exercise compliance(P<0.001).Conclusion:The active cycle of breathing technique combined with 5A model can significantly improve patients' dyspnea,exercise self-efficacy,anxiety and depression,reduce postoperative exercise compliance and pulmonary complications.The active cycle of breathing technique can significantly improve patients' dyspnea,exercise self-efficacy,anxiety and depression,reduce postoperative exercise compliance and pulmonary complications.Compared with the active cycle of breathing technique,the active cycle of breathing technique combined with 5A model has better rehabilitation effect.
