Objective: 1.A clinical randomized controlled trial design was used to observe the effect of 12 weeks of Tai Chi Chuan in young adults with subthreshold depression;2.The effect of Tai Chi Chuan on the gray matter volume of the young adults with subthreshold depression was investigated by using the voxel-based morphology methods,and the possible mechanism of Tai Chi Chuan on the young adults with subthreshold depression was exploredMethods: This study used a randomized controlled clinical trial design.64 eligible subjects were included and randomly divided into two groups.In the Tai Chi Chuan group,32 subjects were received Tai Chi Chuan training.In the blank control group,32 subjects were maintained a routine life,without any intervention training.Tai Chi Chuan training focused on 60 min each time,3 times / week for a total of 12 weeks;the blank control group did not take any intervention to maintain the routine lifestyle.Before and after the intervention,the Hamilton depression scale and Pittsburgh Sleep Quality Index scale were used to evaluate the subject's depression level and sleep quality.SPSS 24.0 software was used for statistical analysis.Two independent sample T tests or Mann-Whitney U test were used to compare the depression level and sleep quality of the two groups of subjects before and after the intervention.When the P value was less than 0.05,there was a statistical difference.MRI scans of the subjects before and after intervention were performed to collect T1 structural image MRI data.On the Matlab 2016 platform,SPM 12 and CAT 12 software were used for data preprocessing and voxel-based morphological analysis(VBM)to compare the differences in gray matter volume(GMV)between the Tai Chi Chuan group and the blank control group before and after intervention.And the difference in gray matter volume between the two groups before and after the intervention and the difference in the total score of the Hamilton Depression Scale between the two groups were analyzed by partial correlation analysis to analyze the correlation between gray matter volume and depression level.Results:Before the intervention,the subjects of the two groups were in terms of gender,age,body mass index(BMI),years of education,total intracranial volume(TIV),parental emotional status,status of the only child,family atmosphere,relationship with parents,professional satisfaction,daily use of mobile phones and monthly living expenses were not significantly different(P>0.05).The total score of the Hamilton depression scale total score and sub-test score before intervention between the two groups of subjects was not statistically significant(P>0.05).There were no significant differences in the total scores and sub-tests in the Pittsburgh Sleep Quality Index Scale before intervention(P>0.05).After the intervention,the difference of the total scores of Hamilton depression scale(P=0.004)and in the sub-test of the scores of somatization of anxiety(P=0.027),block(P=0.014)and sleep disorder(P=0.032)in the two groups were significantly different.However,there was no significant difference in the sub-test score between body weight and cognitive impairment(P>0.05).There were significant differences in the difference between the total scores of the Pittsburgh Sleep Quality Index Scale(P=0.001)and the difference in daytime dysfunction scores(P=0.027)between the two groups of subjects.However,the Pittsburgh Sleep Quality Index Scale sub-test(sleep quality,time to sleep,sleep time,sleep efficiency,sleep disorders and hypnotic)scores were not statistically significant(P>0.05).After VBM analysis,it was found that before and after intervention in the Tai Chi Chuan group,the frontal lobe,anterior cingulate gyrus,right insula and anterior lobe of cerebellum(P=0.001)were statistically significant.And the gray matter volume of the frontal lobe,anterior cingulate gyrus,right insula and anterior lobe of cerebellum were increased.Middle frontal gyrus,inferior frontal gyrus,opercular part of the inferior frontal gyrus,right inferior temporal gyrus,left thalamus,right thalamus,left insula,cerebellar vermis,and posterior lobe of cerebellum were statistically significant(P=0.001).And the gray matter volume of the middle frontal gyrus,inferior frontal gyrus,opercular part of the inferior frontal gyrus,right inferior temporal gyrus,left thalamus,right thalamus,left insula,cerebellar vermis,and posterior lobe of cerebellum were decreased.The blank control group was found before and after the intervention in the frontal lobe,anterior cingulate gyrus,left insula and right insula(P=0.001)were statistically significant.And the gray matter volume of frontal lobe,anterior cingulate gyrus,left insula and right insula were increased.Parietal lobe,temporal lobe,middle temporal gyrus,inferior temporal gyrus,limbic lobe,left thalamus,posterior lobe of cerebellum and cerebellar tonsil(P=0.001)were statistically significant.And the gray matter volume of parietal lobe,temporal lobe,middle temporal gyrus,inferior temporal gyrus,limbic lobe,left thalamus,posterior lobe of cerebellum and cerebellar tonsil were decreased.The difference in gray matter volume between the two groups was in pons,locus coeruleus,raphe nuclei(MNI coordinates:-3,-30,-18;cluster size=405;P=0.001)and cerebellum(MNI coordinates: 2,-60,-12;cluster size=160;P=0.001)has statistical significance.And the gray matter volume of the pons,locus coeruleus,raphe nuclei and cerebellum were increased.Partial correlation analysis found that,the difference in gray matter volume of the Tai Chi Chuan group before and after intervention was positively correlated with the difference in the total score of HAMD-17 in the Tai Chi Chuan group(r=0.519,P=0.007);The difference in gray matter volume of the blank control group before and after the intervention was negatively correlated the difference in the total score of HAMD-17 in the blank control group(r =-0.446,P=0.029);The difference in gray matter volume between the two groups before and after the intervention was negatively correlated with the difference in the total score of the HAMD-17 between the two groups before and after the intervention(r=-0.289,P=0.049).Conclusion: 1.The 12-week Tai Chi Chuan training can improve the depression level and sleep quality of young adults with subthreshold depression.2.Tai Chi Chuan training can increase the gray matter volume of the pons and cerebellum,and then regulate the depression level of young adults with subthreshold depression.
