Effect Of Tibial High Osteotomy-Ilizarov External Fixation Combined With Bushen Huoxue Granule In The Treatment Of Kidney Deficiency And Blood Stasis Type KOA

Objective To investigate the effect of Bushen Huoxue granule on tibial high osteotomy-Ilizarov external fixation in the treatment of knee osteoarthritis with kidney deficiency and blood stasis.Method The 62 patients who met the inclusion criteria were randomly divided into the experimental group and the control group.High tibial osteotomy-Ilizarov external fixation was performed in both groups.The experimental group was treated with Bushen Huoxue granules for 6weeks,3 weeks for a course of treatment.In the morning,preoperative and postoperative peri-knee diameters of the two groups were recorded.NO level in joint fluid was detected before and after treatment(6 weeks after surgery).VAS score,femoral tibial Angle,proximal medial tibial Angle and Lysholm score were observed before,3 months after and 6 months after surgery.Clinical efficacy was evaluated 4 months after surgery.Result1.There was no statistically significant difference between the two groups in preoperative gender,age,kellgren-lawrence(k-l)classification and various observation indicators(P>0.05),which was of comparative research value.2.Comparison of VAS scores: comparison of VAS scores 3 months after surgery between the two groups showed statistically significant difference(P<0.01).There was no statistically significant difference in VAS scores between the two groups 6 months after surgery(P>0.05).There was a statistically significant difference in VAS scores between the two groups 3months after surgery and 3 months after surgery(P < 0.01),and there was a statistically significant difference between the two groups 6 months after surgery and 3 months after surgery(P < 0.01).3.FTA and MPTA comparison: there was no statistically significant difference between the two groups in FTA and MPTA comparison 3 months and 6 months after surgery(P>0.05).The difference between the two groups was statistically significant(P<0.01)when compared with that of FTA and MPTA 3 months after the operation(P<0.01),and that of FTA and MPTA 6months after the operation(P<0.01).4.Comparison of Lysholm scores: comparison of Lysholm scores 3 months and 6 months after surgery between the two groups showed statistically significant differences(P<0.01).There was a statistically significant difference in Lysholm score between the two groups 3months after surgery and before surgery(P<0.01),and there was a statistically significant difference between the two groups 6 months after surgery and before surgery(P<0.01).5.Comparison of knee circumference: there was no statistically significant difference in the comparison of knee circumference between the two groups on day 1 and day 7 after surgery(P>0.05).The comparison of knee circumference on day 3 and day 5 after surgery was statistically significant(P<0.05).6.Comparison of NO content in knee joint fluid: the difference of NO content in knee joint fluid between the two groups after treatment was statistically significant(P<0.01).The content of NO in the knee joint fluid after and before treatment in the two groups was statistically significant(P<0.01).7.Comparison of clinical efficacy: the two groups of patients included in the study achieved good clinical results,with the total effective rate of 83.3% in the control group and 93.3% in the experimental group,showing differences in the total efficacy of the two groups(P<0.05).Conclusion For patients with kidney-deficiency and blood-stasis type KOA who underwent tibial high osteotomy-Ilizarov external fixation,postoperative treatment with Bushen Huoxu granule can effectively improve knee function,relieve pain,reduce the concentration of NO in joint fluid,and promote swelling in the early postoperative period.Its total clinical effective rate is higher,which is worthy of promotion.
