Objective To investigate the risk factors of acute kidney injury(AKI)caused by wasps sting and the factors affecting the prognosis of renal function in patients with AKI.Methods Analysis the general information,clinical manifestations,laboratory tests,treatments,discharge status,and renal function recovery of the inpatients with wasps stings,From January 2013 to December 2019 in Department of Nephrology and Intensive Care Unit of Minda Hospital affiliated to Hubei Minzu University.All the patients were divided into AKI group and non-AKI group according to the occurrence of AKI,inpatients with AKI were divided into good prognosis group and poor prognosis group according to the recovery of renal function.Risk factors of AKI after wasps stings and factors affecting prognosis of renal function were analyzed by Logistic regression.Results 1.Among the 190 patients with wasps stings who met the study criteria,84(44.21%)had AKI,and 106(55.79%)had non-AKI.Among the patients with AKI,30(35.71%)had good renal function prognosis and 54(64.29%)had poor prognosis.There were 16 deaths in AKI group(8.42%)and no deaths in the non-AKI group.2.Analysis of AKI risk factors caused by wasp sting: univariate analysis showed that there were statistically significant differences between AKI group and non-AKI group in gender,number of stings,time from sting to consultation,white blood cells,red blood cells,hemoglobin,lactate dehydrogenase,myoglobin,etc.(P<0.05);Multivariate Logistic regression analysis showed that the number of stings(OR=1.226,95%CI[1.101-1.365]),lactate dehydrogenase(OR=1.003,95%CI[1.000-1.006]),anemia(OR=42.826,95%CI[3.637-504.289]),white blood cells(OR=1.222,95%CI[1.068-1.400]),and male(OR=13.132,95%CI[1.067-161.632])were positively correlated with AKI after wasps sting(P<0.05);3.Analysis of factors affecting renal function prognosis:univariate analysis showed that there were statistically significant differences in the number of stings,age,time from sting to consultation,hemoglobin,lactate dehydrogenase,serum creatinine,myoglobin and urine protein between the groupwith poor prognosis of renal function and the group with good prognosis of renal function(P<0.05).Multivariate Logistic regression showed that the number of stings(OR=1.136,95%CI[1.041-1.240]),age(OR=1.120,95%CI[1.040-1.207])and serum creatinine at admission(OR=1.005,95%CI[1.001-1.010])were positively correlated with poor prognosis of renal function(P<0.05).Conclusion 1.High incidence of AKI caused by wasp stings in this region,AKI patients have a high rate of poor renal function;2.Males,number of stings,anemia,increased white blood cell and lactate dehydrogenase values are independent risk factors for AKI after wasps stings;3.Number of stings,age,and serum creatinine at admission are independent risk factors for poor prognosis of renal function in patients with AKI.
