Objective: With the increase in the obesity rate of adults and the aging of the population,non-alcoholic fatty liver disease(NAFLD)is very common around the world.It is a cause of increasing cirrhosis and hepatocellular carcinoma,and it is increasingly becoming a liver transplant index.Type 2 diabetes(T2DM)is the largest related disease of NAFLD,and the prevalence of combined NAFLD is gradually increasing.This requires public health measures to slow the growth of NAFLD and T2 DM.With the gradual advancement of the national graded diagnosis and treatment model,the awareness of community medical workers on NAFLD and its T2 DM is particularly important.At present,T2 DM has been included in the community chronic disease management project.Therefore,the purpose of this study is to understand the perception of NAFLD by community medical staff,and to explore the risk factors of TFL and NFLD combined with T2 DM,in order to discover the weak links of community medical staff on NAFLD and the risk factors of TFL and NFLD.Develop community medical staff's correct understanding of NAFLD,and then provide a basis for standardizing medical staff's NAFLD diagnosis and treatment plan,and provide new reference standards for early prevention,clinical diagnosis and treatment of patients with T2 DM combined with NAFLD.Methods:(1)A random sample of 39 community health service centers in Xi'an was selected.From January to April 2019,a self-made questionnaire was used to centrally issue to 630 community medical staff,mainly focusing on the concept of NAFLD and epidemiology,risk factors and related diseases,diagnosis,treatment and follow-up of six parts was performed using IBM SPSS24.0 software.The obtained results are mainly described by frequency distribution and percentage.(2)By reviewing the electronic medical records of a provincial people's hospital in Northwest China from June 2018 to June 2019,386 patients with type 2 diabetes diagnosed for the first time were collected,including 172 NAFLD patients and 214 non-NAFLD patients,and recorded their medical history and anthropological indicators.,Biochemical indicators,CT test results and other data,with or without NAFLD as the independent variable,univariate analysis and multiple logistic regression analysis were performed to construct a multiple logistic regression equation to screen out the risk factors for T2 DM combined with NAFLD.Results:(1)A total of 630 questionnaires were distributed and 581 valid questionnaires were obtained,with an effective recovery rate of 92.22%.The rate of correctness of NAFLD awareness among community medical staff in the sample population was as follows: concept and epidemiology(62.48%),treatment(58.87%),diagnosis(43.36%),follow-up and testing(34.15%).Risk factors and related diseases(31%).(2)The prevalence of T2 DM combined with NAFLD was 44.56%(172/386).The detection rate of T2 DM combined with NAFLD with BMI <23kg/m2 was 15.1%.The factors related to T2 DM combined with NAFLD were waist circumference(OR = 1.136,P <0.01),TG(OR = 1.434 P <0.01),BMI(OR = 1.019,P <0.01).Conclusions:(1)Xi'an's community medical staff has a lack of awareness of NAFLD.Among them,the risk factors for NAFLD and its related diseases are the weakest.This directly affects community medical staff's learning and education about this disease,and it is a serious reminder that we should strengthen Reeducation of NAFLD-related knowledge of medical staff,guiding their medical staff to accurately screen,diagnose,evaluate and treat NAFLD patients.(2)BMI,waist circumference,and triglycerides are risk factors for type 2 diabetes with NAFLD.Therefore,the above indicators should be intervened in the early stage of patients with type 2 diabetes,and the prevalence of NAFLD should be reduced through comprehensive treatments such as weight loss,hypoglycemia,and lipid regulation.
