Objective:The increasing prevalence of non-alcoholic fatty liver disease(NAFLD)has become a major global public health problem.Previous studies have shown a correlation between nutritional status and NAFLD.At the same time,subclinical inflammation is related to the prevalence of NAFLD.The main purpose of this study was to investigate the relationship between dietary inflammatory index(DII)and NAFLD in the elderly,and to analyze the dietary inflammatory characteristics of the elderly,so as to provide primary prevention of NAFLD in the elderly from dietary structure and reduce the incidence of NAFLD.Methods:A cross-sectional study was conducted on 468 elderly people aged 60 and over who met the inclusion and exclusion criteria in the health examination of Dongguan South Street Community Health Service Center in Xi'an from June 2019 to November2019.General clinical data,abdominal ultrasound results,routine biochemical test results and DII scores of 468 elderly people were collected.According to the DII score of 468subjects,the diet was divided into a pro-inflammatory diet group and an anti-inflammatory diet group(i.e.,a positive DII score was divided into a pro-inflammatory diet group and a negative DII score was divided into an anti-inflammatory diet group).Chi-square test,Student's t test,Fisher's exact test,kruskal-wallis H non-parametric test,Analysis of Variance(ANOVA),Spearman rank correlation analysis,Logistic regression analysis were used to study the relationship between DII and NAFLD.Results:(1)There were significant differences in body weight,waist circumference,body mass index(BMI),exercise and triglyceride(TG)among different dietary inflammation index groups(P<0.05);The differences of body weight,waist circumference,BMI,exercise,triglyceride,alanine aminotransferase(ALT)and aspartate aminotransferase(AST)in different liver status groups were statistically significant(P<0.05).(2)According to the DII scores of the four groups of liver status group participants(the elderly),the median scores of dietary inflammation index in the normal liver group,the mild NAFLD group,the moderate NAFLD group and the severe NAFLD group were-0.26,2.64,3.95 and 5.93,respectively,with statistically significant differences(?~2=91.143,P<0.05).(3)There was a positive correlation between liver status and DII score(r_s=0.436,P<0.05).After the partial correlation analysis of age,weight,BMI and other factors was adjusted,there was still a positive correlation between liver status and DII score(r=0.331,P<0.05).(4)Binary Logistic regression analysis was conducted with the level of DII score as the covariable and the occurrence of NAFLD as the dependent variable.The results showed that there was a statistical significance between DII score and NAFLD(P<0.05)(Rude OR=1.42,95%CI:1.303-1.548).After adjusting for the relevant influencing factors of NAFLD,the results showed that there was still a statistical significance between DII score and NAFLD(P<0.05)(Adjust OR=1.325,95%CI:1.206-1.456),and the risk of NAFLD increased by 1.325 times for every 1 unit increase in DII score.Conclusions:(1)body weight,waist circumference and BMI were negatively correlated with DII.(2)BMI triglycerides and alanine aminotransferase are independent risk factors for NAFLD.(3)The incidence of NAFLD was higher in the elderly with pro-inflammatory diet than in the elderly with anti-inflammatory diet.(4)DII score was positively correlated with NAFLD in the elderly,and DII score was an independent risk factor for NAFLD.
