OBJECTIVE:To explore the differential gene expression and pathway analysis of specific antibody-related dermatomyositis patients at the level of m RNA,and to find out the possible pathogenesis of dermatomyositis.Dermatomyositis patients often suffer from itching,photosensitivity and other symptoms.TRPV is an ion channel related to heat,light and itching.The expression of TRPV in facial lesions of dermatomyositis patients was studied qualitatively and quantitatively,and the involvement of TRPV in skin was preliminarily studied.The pathogenesis of dermatomyositis lesions provides a new method for the treatment of dermatomyositis lesions.METHODS:According to specific antibodies and complications,dermatomyositis patients were divided into three groups: anti-MDA5 antibody-positive group with pulmonary interstitial disease,anti-TIF1-gamma antibody-positive group with tumor,myositis-specific antibody-negative group without organ involvement.Peripheral blood of 10,8,9 patients in each group and 7 healthy controls were collected.And healthy controls and patients were screened by RNA-seq technique.The differentially expressed genes were verified by q RTPCR and analyzed by GO and Pathway.The facial lesions of 11 dermatomyositis patients and 9 healthy controls were collected.TRPV1,TRPV2,TRPV3,TRPV4 was semiquantitatively and localized by immunohistochemistry,and TRPV1,TRPV2,TRPV3,TRPV4 was quantitatively analyzed by q RT-PCR.RESULTS:Compared with healthy controls,the expression of IFI27,DEFA3,RETN and RNASE2 genes in peripheral blood of dermatomyositis patients was significantly increased(P=0.0001,0.0028,0.0146 and 0.0104,respectively).The analysis of clinical test indicators showed that:(1)DEFA3 and RNASE2 expression in patients with fever was higher than that in patients without fever,the difference was statistically significant,P values were 0.0047 and 0.0002,respectively;(2)DEFA3,RETN and RNASE2 expression in patients with elevated lactate dehydrogenase(LDH)was significantly higher than that in normal LDH group,P values were 0.0397,0.0157 and 0.0209,respectively;(3)DEFA3 and RNASE2 expression in ferritin increased.The expression in high group was higher than that in normal group,the difference was statistically significant,P values were 0.0446 and 0.0103,respectively.In TRPV experiment,immunohistochemistry showed that TRPV1,TRPV2 and TRPV4 was highly expressed in facial lesions of dermatomyositis patients compared with healthy controls,but there was no significant difference in TRPV3 expression.The results of q RT-PCR were consistent with those of immunohistochemistry.The P values of TRPV1,TRPV2,TRRPV4 were 0.0005,0.0012 and 0.0004 respectively.CONCLUSION:Increased expression of IFI27,DEFA3,RETN and RNASE2 may be involved in the pathogenesis of dermatomyositis,especially in serum anti-MDA5 antibody positive DM/CADM.DEFA3 and RNASE2 are potential diagnostic markers for DM/CADM combined with acute ILD(A/SIP).The high expression of TPRV1,TRPV2 and TRPV4 in dermatomyositis lesions suggests that TPRV1,TRPV2 and TRPV4 may be involved in the occurrence of dermatomyositis lesions,which provides a new idea for the treatment of dermatomyositis lesions and may be used as new therapeutic targets in clinic.
