Objective Lung cancer is the leading cause of cancer-related death,with non-small cell lung cancer accounting for 85% of the total incidence.The popularity of highresolution CT has increased the detection rate of pulmonary nodules.However,the diagnosis of pulmonary nodules mainly relies on high-cost,traumatic needle biopsy technology,and there is an urgent need for rapid,efficient and low-cost detection technology.Circulating DNA is a hot field in cancer research.Studies have confirmed that free DNA(Cell-Free DNA,cf DNA)exists mainly in the form of nucleosomes.Currently,there are few studies on circulating nucleosomes.This study will explore the feasibility of the body as a diagnostic marker for pulmonary nodules.Methods The plasma nucleus of 70 patients with pulmonary nodule surgery was enriched by the improved ultra-low input immunoprecipitation technique.To compare the circulating nucleosome monomer,oligonucleosome concentration and apoptotic index in patients with benign and malignant pulmonary nodules,and to explore the feasibility of circulating nucleosome as a diagnostic marker for pulmonary nodules.Compare with cf DNA and explore the circulating nucleus.The value of the corpuscle as a potential diagnostic marker.Results The average levels of plasma H3K4me3 circulating nucleosomes in patients with NSCLC and pulmonary benign disease were 0.54±0.70ng/ml and 0.18±0.18ng/ml,respectively.The average content of oligomeric nucleus was 0.43±0.94ng/ml,0.11±0.08ng/ml,the difference between patients with benign and malignant nodules was statistically significant(P<0.01).H3K4me3 nucleosome monomer concentration,H3K4me3 oligonucleoside concentration and cf DNA apoptotic rate(AI)can be used as potential diagnostic indicators.The critical values are 0.255 ng/ml,0.155 ng/ml,8.42,respectively.The sensitivity is 0.627,0.569,1;the corresponding specificities are: 0.895,0.842,0.549.The combined H3K4me3 nucleosome concentration,cf DNA AI index sensitivity can be increased to 0.765,and the specificity can be increased to 0.947.The nucleosome apoptotic index,oligonucleoside concentration and positive AJCC staging were positively and negatively correlated,and the correlation was statistically significant(P=0.049,P=0.017).The concentration of H3K4me3 nucleosome was correlated with the nipple component of lung adenocarcinoma(P=0.032).Conclusion Through the comparative study of benign and malignant nodules in the lung,it is found that the circulating nucleosome has potential application value in the diagnosis and pathological type identification of malignant tumor.The combination of circulating nucleosome level and cf DNA apoptotic index applied to the lung node in good and malignant judgments can be achieved better than traditional cf DNA testing.
