Objective: The metabolic syndrome,a concurrence of disturbed glucose and insulin metabolism,overweight and abdominal fat distribution,dyslipidemia,and hypertension,is associated with poor prognosis and recurrence of stroke.Mechanical thrombectomy is effective as a promising therapeutic approach for acute ischemic stroke,the prognosis of patients with thrombectomy is associated with many factors,the impact of metabolic syndrome on prognosis among those with anterior circulation thrombectomy is less clear.This study was aimed at investigating the effect of MS on the prognosis of patients with anterior circulation thrombectomy,to provide evidence for improving the prognosis of patients with thrombectomy.Methods: Patients included in this analysis were those who were enrolled in "Chinese Stroke Center Alliance acute ischemic stroke endovascular treatment collaborative group(ANGEL)".Patients were divided into MS and non-MS group according to the diagnosis based on metabolic syndrome,compare the difference of prognosis between the two groups,90 days poor functional outcomes(modified Rankin Scale ? 3)is the primary efficacy endpoint;90 days ischemic stroke recurrence is the secondary efficacy endpoint.A Unconditioned logistic regression analysis was used to identify variables influencing clinical outcome,based on m RS at 3 months,investigate the poor prognosis correlation with metabolic Syndrome after mechanical thrombectomy.Kaplan-memier method was used to analysis ischemic stroke recurrence difference in the two groups.Results:A total of 159 patients who received mechanical thrombectomy were included in this study,40 and 24.52% in MS group,119 and 75.48% in non-MS group,66 and 41.51% had poor prognosis,4 and 2.52% had ischemic stroke recurrent.After adjusted for confounders including age,gender,atrial fibrillation,the unconditioned logistic regression analysis showed that MS had a high risk of poor functional outcomes than non-MS[(Odds Ratio,OR)=2.658,95%confidence interval(CI)1.247~5.666],patients with MS may are not at higher risk than non-MS in the recurrence of ischemic stroke 90 days after thrombectomy.Conclusion : MS may increase the risk of poor prognosis in patients with mechanical thrombectomy.It is necessary to develop individualized treatment plan to improve the quality of life of patients with metabolic syndrome who receive mechanical thrombectomy.
