Objective:to study the significance of several new inflammatory indexes in peritoneal dialysis-peritonitis,so as to understand the inflammatory state during the occurrence of clinical peritoneal dialysis-associated peritonitis. It has some suggestion to the severity of the disease, treatment effect and prognosis. Methods:Retrospective analysis was made on continuous ambulatory peritoneal dialysis(CAPD)of 122 patients from January 1,2017 to December 31,2019 in the Department of Nephrology,Third Hospital of Hebei Medical University. The patients were divided into peritoneal dialysis related peritonitis group(n=62)and normal peritoneal dialysis group(n=60),including peritoneal dialysis related peritonitis group was divided into before the treatment group and after the treatment which was 3-7days.The difference of ratio of mononuclear cells to lymphocytes was compared between the two group, the influencing factors of peritonitis with peritoneal dialysis were analyzed, the relationship between MLR and other clinical indicator such as NLR?PLR?serum albumin, hemoglobin?urea?creatinine?electrolytes, and the changes of clinical indicators before and after treatment. The Mann-whiteney U test and t test were used for statistical analysis. Results: 1.Compared with the normal peritoneal dialysis group, the white blood cell count?neutrophil count and NLR?PLR?MLR were higher in the peritoneal dialysis-associated peritonitis group(P<0.05).the lymphocyte count? hemoglobin? serum albumin?blood potassium,?blood sodium? blood phosphorus and blood magnesium were lower, and the difference between the two groups was statistically significant.2.Compared with the normal peritoneal dialysis group?there was no significant difference between the two groups: dialysate?age?BMI?monocyte count?platelet count,?blood urea?serum creatinine?corrected calcium?blood uric acid.Spearman correlation analysis was used to show NLR and dialysate?white blood cell count? neutrophil count? monocyte count and PLR?MLR?PCT?CRP were positivelycorrelated with lymphocyte count, blood sodium and blood magnesium.PLR was positively correlated with dialysate?neutrophil count?platelet count? NLR?MLR and negatively correlated with age?lymphocyte count?urea.MLR was positively correlated with dialysate?leukocyte count?neutrophil count? monocyte count?PLR?NLR?PCT?CRP and negatively correlated with lymphocyte count? blood sodium? blood magnesium.Analysis of Peritonitis Occurrence and Laboratory Finger by Spearman Correlation Analysis marker correlation showed that the occurrence of peritonitis was positively correlated with leukocyte count?neutrophil count?Spearman correlation analysis was used to analyze the correlation between peritonitis occurrence and each laboratory index. The occurrence of peritonitis showed positive correlation with white blood cell count?neutrophil count,?NLR?PLR?MLR?CRP and negative correlation with lymphocyte count?hemoglobin?serum albumin?blood potassium?blood sodium?blood uric acid?blood phosphorus?blood magnesium.Pair samples were used before and after treatment t the test showed that the lymphocyte count, blood sodium were higher than after treatment,NLR?PLR?MLR?urea was lower than after treatment, and the difference was statistically significant.Detection of serum albumin by Logistic regression analysis white,NLR?hemoglobin and blood phosphorus are closely related to peritoneal dialysis-related peritonitis and it may be independent risk factors. Conclusions: 1.MLR?NLR like traditional inflammatory indicators, could be used as a new indicator to reflect the inflammatory state of peritoneal dialysis-related peritonitis, and PLR and NLR can be used as an indicator to reflect the therapeutic effect of peritonitis.2.The occurence of peritoneal dialysis-related peritonitis is closely related to serum albumin,NLR,hemoglobin and blood phosphorus, and it may be an independent risk factor.
