Background:Stroke is a common and frequently-occurring disease of the nervous system,with high morbidity and mortality.In recent years,it ranks 1st and 2nd in the death order of Chinese population,while ischemic stroke accounts for 60%?80%of total stroke[1].Early diagnosis and evaluation of the prognosis of acute ischemic stroke is conducive to the early initiation of acute ischemic stroke treatment and provide a basis for the evaluation of the value of rehabilitation training.Atherosclerosis?AS?is the main pathological basis of stroke,and its essence is vascular inflammatory disease.Currently known inflammatory markers associated with atherosclerosis are mainly Chlamydia pneumoniae,mononuclear macrophages,C-reactive protein,and all.However,the relationship between neutrophil gelatinase-associated lipocalin and calcitonin and atherosclerosis and acute ischemic stroke remains unclear.Objective:To study the correlation between neutropil gelatinase-associated lipocalin?N-gal?,calcitonin?CT?and C-reactive protein?CRP?with acute ischemic stroke.Evaluate the prognosis of ischemic stroke by the above indicators,and discover the clinical application value.Methods:Prospective collection of 53 patients with acute cerebral infarction as observation group and 53 patients without intracranial lesions during the same period as a control group.Collecting the fasting blood samples of observation group within24 hours in hospital and one week later,and the fasting blood samples of control group,to detect the level of N-gal?CT and CRP separately.Cervical vascular ultrasonography and head MRI examination were performed on the observation group and the control group.Assessing the NIHSS scores within 24 hours in hospital and one week to the observation group,and to compare with them.Results:The N-gal value and CRP value of the observation group were significantly higher than those of the control group?P<0.05?,no statistical difference was found in the CT value on the observation group and the control group?P>0.05?.The ROC curve found that CRP can distinguish acute cerebral infarction and no intracranial lesions with high sensitivity and specificity,and the best diagnostic value is 1.7mg/L.There was a positive correlation between N-gal value and CRP value in the observation group and carotid artery and vertebral artery plaque?P<0.05?,and there was no correlation between CT and carotid artery and vertebral artery plaque?P>0.05?.There were no correlations between N-gal value,CT value and CRP value of the observation group and the size of acute ischemic stroke?P>0.05?.The N-gal value and CRP value of patients with acute ischemic stroke within 24 hours in hospital were significantly higher than after one-week later?P<0.05?,no statistical difference was found in the CT values on patients with acute ischemic stroke within24 hours in hospital and one week later?P>0.05?.In the observation group,the N-gal difference within 24 hours in hospital and one week later was negatively correlated with the difference in NIHSS scores within 24 hours in hospital and one week later?P<0.05,r=-0.289?,no statistical difference in the observation group was found between the difference of CT and CRP within 24 hours in hospital and one week later and the difference of NIHSS score within 24 hours in hospital and one week later?P>0.05?.Conclusion:N-gal and CRP are associated with carotid plaques,vertebral artery plaques and acute ischemic stroke,N-gal is of great value in the diagnosis and prognosis assessment of acute ischemic stroke.
