Objective:To observe the therapeutic effect of acupoint catgut embedding on patients with Combined Allergic Rhinitis and Asthma Syndrome?CARAS?,and to investigate the mechanism of action by acupoint catgut embedding on rats with allergic rhinitis-asthma syndrome.Methods:Part I Clinical Research:The patients were admitted to the Otolaryngology Clinic and Respiratory Clinic of Affiliated Hospital of Chengdu University of Traditional Chinese Medicine from February 2019 to November 2019,and met the diagnostic criteria of Western medicine allergic rhinitis-asthma syndrome and the Chinese medicine The diagnosis of"disease"and"nasal effusion"belongs to patients with mild to moderate bronchial asthma whose severity grade is in the non-clinical remission period.Using prospective randomized controlled research methods,subjects were randomly divided into two groups.The control group was asked to inhale powder included Budesonide and Formoterol and to take Montelukast Sodium tablets.On the basis of the former,the treatment group was combined with bilateral embedding therapy of Yingxiang,Feishu and Dingchuan.The embedding of acupoints was given every two weeks for a total of 8 weeks.The asthma control test score?ACT?and nasal conjunctivitis quality of life questionnaire score?RQLQ?of the two groups before treatment,the fourth week of treatment,and the eighth week of treatment were collected to evaluate the clinical efficacy of acupoint catgut embedding therapy for combined allergic rhinitis-asthma syndrome.Part II Animal experiment:42 SD rats were randomly divided into blank control group,allergic rhinitis-asthma syndrome model group,and embedding intervention treatment group,14 rats in each group.The model group and the treatment group used intraperitoneal injection combined with aerosol inhalation and nasal instillation of ovalbumin to induce combined allergic rhinitis and asthma syndrome model.After the model was formed,the allergic rhinitis-asthma syndrome group did not intervene after the model was built.The line intervention treatment group was given bilateral embedding therapy at Yingxiang,Feishu and Dingchuan points every 2 weeks for 4 times.The blank control group was not given any intervention or treatment.After the experiment,the rats were sacrificed and hematoxylin-eosin?HE?staining was used to observe the morphological changes of lung tissue in each group of rats.Serum was taken for ELISA to detect the difference of serum TGF-?1 and IL-4 levels in each group of rats.Results:The first part of the clinical study:?1?Differences in RQLQ scores within and between groups:The RQLQ scores of the control group and the treatment group were significantly lower than those before treatment at weeks 4 and 8,and the differences were statistically significant.In the fourth week of treatment,although the RQLQ score of the treatment group was lower than that of the control group,the difference was not statistically significant.At the 8th week of treatment,the RQLQ score of the treatment group was significantly lower than that of the control group,the difference was statistically significant;?2?Differences in ACT scores between and within the group:the ACT scores of the control group and the treatment group after treatment were higher than before treatment,and The number of fully controlled persons increased,while the number of uncontrolled persons decreased significantly.The difference was statistically significant.At the 4th and 8th week of treatment,the ACT score of the treatment group was higher than that of the control group,and the difference was statistically significant.The second part of the animal experiment:?1?HE staining results:compared with the blank group,the CARAS model group lung tissue inflammatory cell infiltration significantly increased,and there are varying degrees of airway structure changes,embedding treatment can appropriately reduce lung tissue inflammation Cell invasion and airway structure change,but its severity is still higher than the blank group.?2?ELISA examination of serum IL-4 and TGF-?1differences in each group:The concentration of IL-4 and TGF-?1 in serum of rats in CARAS group was significantly higher than that in blank group,and the differences were statistically significant?P<0.05?),The serum IL-4 concentration and TGF-?1concentration in the embedding group were not significantly different from the blank group.Compared with the CARAS group,the serum IL-4 concentration of the rats in the embedding group decreased,the difference was statistically significant,and although the serum TGF-?1 concentration was lower than the CARAS group,the difference was not statistically significant.Conclusion:Catgut embedding at acupoints has a good effect on alleviating the symptoms of allergic rhinitis-asthma syndrome,which may be related to the reduction of inflammatory factors.
