Objective: Intracranial arterial stenosis is an important cause of ischemic cerebrovascular disease, which often brings heavy burden to families and society due to its high mortality and disability rate. However, asymptomatic extracranial artery stenosis often fails to attract attention due to its clinical asymptomatic or mild symptoms,so early recognition and prevention are crucial.Based on the theory of Chinese medicine to treat diseases, this paper discusses the correlation between asymptomatic extracranial artery stenosis and the constitution of traditional Chinese medicine,in order to provide theoretical basis for the prevention and treatment of asymptomatic extracranial artery stenosis.Method: Subjects were determined according to inclusion and exclusion criteria, general conditions, clinical data and imaging data of the patients were collected, and the included medical records were classified according to the TCM constitution classification and determination scale through clinical questionnaire.SPSS 25.0 was used for statistical analysis.Results:1.109 cases of asymptomatic extracranial artery stenosis included 63 males(57.80%)and 46 females(42.20%).The proportion of males and females =1.37,and the chi-square test P=0.103 > 0.05,indicating that there was no statistical difference in the gender distribution of asymptomatic patients with intracranial arterial stenosis. The age range of the included cases was 41-77 years old, with an average age of 59.74±9.07 years old, including 10 young people,50 middle-aged people and 49 elderly people. The chi-square test showed that P=0.000 < 0.01,indicating that the age distribution of asymptomatic patients with intracranial arterial stenosis was statistically significant, and the majority of the patients were middle-aged and elderly(P < 0.01).2.The distribution of bmi in asymptomatic patients with intracranial arterialstenosis was statistically significant, and the distribution of normal bmi and overweight patients was more(P < 0.01).3.The gender distribution of patients with hypertension, smoking and alcohol consumption was statistically significant, with more male patients than female patients(P < 0.05).The gender distribution of patients with diabetes, hyperlipidemia and overweight and obesity was not statistically significant.4.There were statistically significant differences in age distribution among patients with diabetes, hypertension, hyperlipidemia, smoking, drinking, and overweight and obesity, and the distribution was mostly among middle-aged and elderly patients(P < 0.05).5.Among the 109 patients, there were only 65(59.63%)cases of intracranial artery stenosis,17(15.60%)cases of extracranial artery stenosis, and 27(24.77%)cases of extracranial artery stenosis.By chi-square test, the distribution of stenosis vessels in asymptomatic patients with intracranial artery stenosis was statistically significant, and the majority of them were intracranial artery stenosis(P < 0.01).There were 136 stenosis vessels, including 92 intracranial stenosis and 44 extracranial stenosis. The number of vessels with mild, moderate and severely occluded intracranial vessels was 34,45 and 13,accounting for 36.96%,48.91% and 14.13%,respectively.The number of vessels with mild, moderate, and severely occlusive extracranial vessels was 12,24,and 8,respectively,accounting for 27.27%,54.55%,and 15.44%,respectively.The degree of stenosis is mainly light to moderate.6.The high incidence sites of intracranial arterial stenosis were MCA,i-ica and PCA,accounting for 29.27%(24/82),26.83%(22/82)and 23.17%(19/82),respectively.BCCA was the most common site of extracranial artery stenosis, accounting for 36.96%(34/92).7.The distribution of stenosis in the internal carotid artery system(anterior circulation)and vertebro-basilar artery system(posterior circulation)was statistically significant, and the former circulation was dominant(P < 0.01).In the distribution of stenosis degree, the distribution of anterior and posterior circulation was mainly light to moderate stenosis, and the distribution of anterior and posterior circulation wasmainly moderate to severe stenosis.8.The distribution of single constitution and concurrent constitution in asymptomatic patients with intracranial external artery stenosis was statistically significant, and the distribution of single constitution was dominant(P < 0.01).9.There were statistically significant differences in the accumulative distribution of 9 single constituties.among them, phlegm and dampness, blood stasis and qi deficiency were more distributed, accounting for 28.39%,20.65% and 18.06% of the total body mass, and the distribution of characteristic and mild mass was less, accounting for 1.29% and 1.94% of the total body mass, respectively(P < 0.01).10.The major physical distribution differences of asymptomatic patients with intracranial external artery stenosis were statistically significant, among which phlegm and dampness, blood stasis and qi deficiency were more distributed, accounting for 26.61%,22.94% and 22.02%,respectively,while qi stagnation and characteristic characteristics were less distributed, accounting for 1.83% and 0.92%,respectively(P < 0.01).11.The gender distribution of different constitution types in asymptomatic patients with intracranial external artery stenosis was statistically significant, among which phlegm and dampness were the main body types in males and blood stasis was the main body types in females(P=0.05).12.The distribution of TCM constitution in different age groups of asymptomatic patients with intracranial external artery stenosis was statistically significant.Among them, the distribution of qi deficiency, phlegm and blood stasis was the most in middle and old people, and the least in young people(P < 0.05).13.There was no statistical significance in the distribution of different constitution types and intracranial vascular stenosis.14.The distribution of the anterior and posterior circulation stenosis of different constitution types was statistically significant, and the distribution in the anterior circulation was dominated by Yang deficiency and phlegm and dampness, accounting for 80.00% and 75.86% respectively. The post-circulation was mainly composed of Yin deficiency,qi deficiency and heat and moisture, accounting for 33.33%.Bloodstasis was the dominant factor in the anteropotential circulation, accounting for40.00%(P < 0.01).15.Different constitutions had statistically significant degrees of intracranial arterial stenosis, and the results showed that the mild constitutions were dominated by Yang deficiency,Yin deficiency and qi deficiency, accounting for 70.00%,66.67%and 50.00 % respectively.Blood stasis and phlegm and dampness were the main components, accounting for 63.16% and 58.54% respectively. The proportion of severe-occlusion was 25.00% and 23.68%(P < 0.01).conclusion:1.The majority of asymptomatic patients with intracranial arterial stenosis were middle-aged and elderly patients, and the gender distribution was mainly male.2.The constitutions of asymptomatic patients with intracranial external artery stenosis were mostly distributed as single constitutions;Among the 9 main constitution types, phlegm and dampness, blood stasis and qi deficiency are the most widely distributed.3.Patients with asymptomatic extracranial artery stenosis were mainly confined to intracranial artery stenosis, which was mainly distributed in the internal carotid artery system(anterior circulation).4.In patients with asymptomatic extracranial artery stenosis, intracranial stenosis of the middle cerebral artery, intracranial segment of the internal carotid artery and posterior cerebral artery were more common.Extracranial common carotid artery bifurcation stenosis is common.5.The distribution of pre-circulation constitution was dominated by Yang deficiency, phlegm and dampness,Yin deficiency,qi deficiency and dampness and heat, and blood stasis.6.Asymptomatic extracranial artery stenosis was mainly mild to moderate, and the types of mild stenosis were mainly Yang deficiency,Yin deficiency and qi deficiency.Moderate stricture was mainly characterized by phlegm and dampness and blood stasis.Severe-occlusion is dominated by moist, hot and blood stasis.
