Observe Under The�Invigorating The Spleen And Nourishing The Kidney �for Low Word Recognition Score Deafness Patients' Clinical Efficacy And Analyze The Relevant Factors

Objective : To compare the clinical efficacy of Invigorating the spleen and nourishing the kidney combined with acupuncture in the treatment of patients with hearing loss and low speech recognition score.To explore the clinical significance of using invigorating the spleen and nourishing the kidney treatment method,combined with acupuncture and drugs,to improve hearing in patients with low speech recognition hearing loss and to discuss the factors that may affect the prognosis,to provide a new diagnosis and treatment scheme for patients with low speech recognition hearing loss.Methods: According to the diagnostic standards of Chinese and western medicine,patients with low speech recognition hearing loss who met the inclusion criteria were recorded from Sichuan province Chinese medicine hospital,Emei city Chinese medicine hospital and Sichuan university hospital of Wangjiang campus from June2017 to December 2019 in a randomized,controlled trial.The patients were randomly divided into three test groups: an experimental group,control group 1,and control group 2.The experimental group had a total of 68 patients and 82 affected ears were treated with acupuncture and drugs.Control group 1 had a total of 67 patients and 80 affected ears received acupuncture and moxibustion treatment.Control group 2 with a total of 68 patients and 80 affected ears received traditional Chinese medicine treatment.Case Report Form(CRF)were filled,audiological examination,speech recognition rate examination and evaluation of associated symptoms were completed at four-time points: day 0,2 weeks,4 weeks and 6 weeks after treatment.To observe the differences in the rates of hearing and speech recognition and the factors that may affect the outcome.Finally,SPSS 21.0 software was used to analyze the data.Result : A total of 252 patients with low speech recognition hearing loss were collected in this study,of which 49 cases withdrew and terminated from research.A total of 203 cases were eventually collected,including 101 males and 102 females.There were 164 cases with one ear affected,39 cases with both ears affected,20 cases(22 ears)between 18 and 25 years old patients,26 ~ 35 patients with 25 cases(28ears),30 patients with 36 ~ 45 years old(ears)33,46 ~ 32 cases of 55 patients(36ears),40 patients with 56 to 65 years(52 ears),66 ~ 75 years old patients and 56 cases(71 ears),the onset time in 15 ~ 30 days,a total of 24 ears,onset time in 31 to 60 days,a total of 38 ears,onset time in 61 ~ 90 days,a total of 84 ears,onset time ? 91 days,a total of 96 ears.42 ears with mild hearing loss,accounted for 17.35% of the total affected ears;moderate hearing loss of 53 ears;severe hearing loss of 77 ears,and extremely severe hearing loss of 70 ears.There were a total of 162 cases were accompanied by tinnitus,a total of 41 cases were not accompanied by tinnitus,a total of 59 cases were accompanied by vertigo,a total of 144 cases were not accompanied by vertigo,a total of 132 cases were accompanied by ear stuffiness,a total of 71 cases were not accompanied by ear stuffiness,a total of 130 cases were accompanied by insomnia and a total of 73 cases were not accompanied by insomnia.Chi-square test showed that the baseline data(gender,ear lesion,degree of hearing loss,degree of speech recognition loss,etc.)of each group were not statistically different(P-value >0.05),and can be compared with each other.Age,course of the disease,degree of hearing loss,whether accompanied by tinnitus,ear tightness,vertigo,or insomnia were related to the prognosis of "low speech recognition hearing loss",and other factors such as gender,side of the affected ear,and others have no significant correlation with prognosis.Conclusion:1.The method of �invigorating the spleen and nourishing the kidney� can improve the hearing and speech recognition scores of patients with low speech recognition hearing loss.2.The method of �invigorating the spleen and nourishing the kidney� has a stable effect on the improvement of hearing and speech recognition rate in patients with low speech recognition deafness.3.The method of "invigorating the spleen and nourishing kidney" can reduce the scores of the Report of Communication Apprehension(PRCA-24)and Communication Ability Assessment Scale in patients with "low speech recognition hearing loss",and improve their communication ability and quality of life.4.Discussion of prognostic factors: The analysis results of prognostic factors indicate the age of onset,course of the disease,degree of hearing loss,whether it is accompanied by tinnitus,ear stuffiness,vertigo,insomnia,etc.depends on the prognosis of "low speech recognition hearing loss"(P < 0.05).This shows that it has an impact on the prognosis of "low speech recognition hearing loss".There was no significant correlation between gender,side of the affected ear,and other factors and prognosis(P > 0.05).It can be seen that within the scope of the study,the younger the age,the shorter the course of the disease,the lower the degree of hearing loss,the absence of tinnitus,ear stuffiness,vertigo,and insomnia have a better prognosis.
